- Specifications for Bread and Bread Products West Palm Beach VA Medical Center REQUEST FOR ANNUAL SERVICE AGREEMENT ON BREAD AND BREAD PRODUCTS. SERVICES CONSIST OF TWICE PER WEEK DELIVERIES, AS WELL AS AVAILABILITY FOR EMERGENCY DELIVERIES. All proposals/bids, Appendix A, must be received by E-mail by David Guley by 3 PM EST August 9, 2011 for consideration. All samples must be received by Kathleen Waddell by 3 PM EST August 9, 2011 for consideration. No late proposals/bids or samples after the closing date and time will be accepted. All questions must be submitted by E-mail to david.guley@va.gov by 3 PM August 3, 2011. BASE SERVICE PERIOD WILL BE FROM OCTOBER 1, 2011 TO JUNE 30, 2012 *BASE YEAR CONTRACT WITH FOUR CONSECUTIVE ONE YEAR OPTION PERIODS. 1.OVERVIEW: The contractor shall provide bread and bread products to the VA Medical Center (hereafter known as the VA) located at 7305 North Military Trail, West Palm Beach, Florida, 33410-6400. The VA intends to award a firm, fixed price contract for the provision of bread and bread products. The quantities listed in the attached Pricing Schedule Appendix A are estimated requirements for the Nutrition & Food Services and the Veterans Canteen Service. 2.SCOPE OF WORK: A.General: The scope of this requirement involves providing bead and bread products, which conform to the specifications, outlined and referenced herein, to the VA's Nutrition & Food Service, located at the VA at 7305 N. Military Trail, West Palm Beach, Fl. B.Quantities: The quantities listed in the attached Schedule represent the estimated quantities that will be ordered over a 12-month period. The actual quantity ordered may vary from this estimate but shall not exceed the total estimated quantity for the 12-month period without the prior written approval of the Contracting Officer. C.Period of Performance: The period of performance for the provision of bread and bread products shall be a 8-month base year period from October 1, 2011 - June 30, 2012, with Four 12-month option periods, from July 1, 2012 through June 30, 2016, which may be exercised at the Government's discretion. D.Maintaining Par Levels: The VA shall be responsible for maintaining a par level for each bread product at the Nutrition & Food Services. E.Orders: Bread orders will be submitted twice each week, with 48 hours notice, with the Government reserving the right to adjust orders 24-hours prior to scheduled delivery. Ice cream orders will be submitted weekly, as needed, with 48 hours notice and with the Government reserving the right to adjust orders 24 hours prior to schedule delivery. The Contracting Officer or his/her designee will place orders. Telephone, oral, and wire transmission orders are authorized. F.Deliveries and Invoicing: Deliveries shall occur twice per week between the hours of 7:00 a.m. and 9:30 a.m. Monday and Thursday. VA Points of Contact: The designated Contracting Officer for this requirement is David Guley and the Technical Representatives for this contract is Kathleen Waddell RECOGNIZED FEDERAL HOLIDAYS: New Year's Day Martin Luther King Day Presidents Day Memorial Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Christmas Deliveries will not be accepted on federal holidays. Normally scheduled deliveries that fall on a federal holiday shall be scheduled by mutual agreement for 1-2 business days preceding or following the holiday. 3.HURRICANES/DISASTERS/EMERGENCIES: The contractor must be able to ensure delivery to the VA at all times, inclusive of hurricane, disasters, and/or any other emergency condition. The VA may place a special order in preparation for, or in response to, a disaster or emergency situation, including but not limited to a hurricane. For anticipatory situations, such orders will be placed 24 hours or more in advance. In an emergency, the contractor will be notified with the amount and type of products required by the VA that shall be delivered within 24-48 hours of order placement. The order may be placed for larger quantities than the normal weekly order. The contractor shall provide the Contracting Officer and both technical representatives with its designated point of contact and contact information for emergency orders. 4.SPECIFICATIONS AND STANDARDS: A. The delivered products shall comply with all applicable Federal and State mandatory requirements and regulations relating to the preparation, packaging, labeling, storage, distribution, and sales of the products with the commercial marketplace. All deliveries shall conform in every respect to the provisions of the Federal Food, Drug and Cosmetic Act and regulations, promulgated hereunder. B. Bread products shall have sufficient fiber content per slice, or serving at a minimum: (1)White bread must be a minimum of 3 grams of fiber per slice. (2)100% Whole Wheat bread must be a minimum of 3 grams of fiber per slice. (3)100% Whole Wheat Hamburger and Hot Dog buns must be a minimum of 4 grams of fiber per roll. C. Contractor's Certification. By submitting an offer, the contractor certifies that the product offered meets the specified salient characteristics and requirements of the Federal Hospital Subsistence Guide; conforms to the producer's own specifications and standards, including product characteristics, manufacturing procedures, quality control procedures, and storage and handling territories, or possessions; and is sold in the commercial market. The Government reserves the right to determine proof of such conformance prior to the first delivery from point of origin and any time thereafter, up to and including, delivery at final destination as may be necessary to determine conformance with the terms of the contract. 6. SANITATION AND WORKMANSHIP: All products furnished shall be prepared, processed and packed under sanitary conditions and in accordance with good commercial practice. All containers shall be clean, sound and securely covered or sealed to provide adequate protection from dirt, filth, and all other contamination. 7. FRESHNESS REQUIREMENT FOR BREAD PRODUCTS: A. Bread products shall have sufficient remaining shelf life commensurate with good commercial practice at a minimum: (1)White bread must be delivered a minimum of 5 days prior to the expiration date. (2)Wheat bread must be delivered a minimum of 5 days prior to the expiration date. (3)Hamburg buns must be delivered a minimum of 5 days prior to the expiration date. (4)Hotdog buns must be delivered a minimum of 5 days prior to the expiration date. 8. PACKAGING, PACKING, AND LABELING: A.Unless otherwise specified, preservation, packaging, and packing shall be to a degree of protection to preclude damage to containers and/or contents thereof under normal shipping conditions, handling, etc., shall conform to normal commercial practices and applicable carrier rules and regulations, involving shipment from the contractor to the receiving activity for storage. Shipping containers shall be in compliance with national Motor Freight Classification and Uniform Freight classification (issue in effect at time of shipment). B.All packaging and packing shall be in accordance with good commercial practice. Labeling shall be in accordance with commercial labeling, complying with the Federal Food, Drug and Cosmetic Act and regulations promulgated there under. C.All items must be identified with readable dates (open code dates), or code dates. Contractors who do not use open dating will provide a product code number key listing. The product code number key listing shall explain the actual date of production or processing. A copy of the key-code listing will be furnished to the receiving officer and each inspection agency with the first delivery D.Source of Manufacture: Items supplied under this contract are required to originate from a sanitarily approved source. The supplier must identify the source of manufacture on the primary container or in the accompanying shipping documents. The identification on the primary container may consist of either the manufacturing plant name and address or a manufacturing plant code. When the manufacturing plant is identified on the shipping documents accompanying the delivery, the plant shall be identified by its complete name, address and plant code. E.If it is determined that a significant number of products are spoiled, or otherwise found to not be in accordance with the specifications contained herein, the Contractor will be notified to exchange the products for ones that are in accordance with these specifications within two hours of notification. If the number of such products is determined to be insignificant in number, they may be held for pick up by the contractor at the next scheduled delivery. This solicitation should include the Specifications, Appendix A, and Evaluation Criteria/Instructions to Offerors documents. All bids received under this RFQ will be evaluated by the following criteria with descending order of importance: (1)Technical Capability (2)Past Performance (3)Price Technical capability when combined is more important than price. Award will be made to the offeror whose proposal represents best value to the Government. The winning company must be registered in the Central Contractor Registration System (CCR) https://www.bpn.gov/ccr/default.aspx at the time of award. Note: All documents submitted must not exceed 9 MB in size or they will not be accepted and will not be compatible with VA Information Systems. Instructions to Offerors Offerors shall provide proposals in the following format: (1)Technical capability limited to 10 pages in length and volume Submit a technical performance plan that describes how you will complete the requirements of the Statement of Work (SOW) within schedule. Include a listing of all equipment that will be used to complete the requirements of the SOW. All samples of each requested item should be sent directly to Nutrition and Food Service, Mail code (120), VA Medical Center, 7305 N. Military Trail, West Palm Beach, FL 33410-6400 and to the attention of Kathleen Waddell. (2)Past performance limited to 3 pages total in length and volume Submit 3 references with current names, phone numbers, and e-mail addresses for all efforts similar in size and complexity. (3)Price Submit completed firm fixed price by line item for all items. Indicate if line item is on GSA schedule (G) or Open market (O) for all items
