Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement is solicited under FAR Part 13, with offers evaluated in accordance with FAR 13.106-2. The basis on which the award will be made is technical, past performance and price. The combined synopsis/solicitation number is VA-263-11-RQ- 0529. The incorporated provisions and clauses herein are in effect through Federal Acquisition Circular 200-53 effective 07/05/2011. THIS REQUIREMENT IS 100% SET-ASIDE FOR SMALL BUSINESS. The associated NAICS is 621399 with a size standard of $7 Million. The contractor shall provide all labor, supervision, materials, equipment and supplies necessary to provide VASQIP Clinical Reviewer Services, see Performance Work Statement (Attachment 1). There are five line items with a period of performance of one year (365 days) for the base year, with four twelve month option years as follows: Line Item 0001: Base Period (1 Sep 2011 - 31 Aug 2012): Services of Clinical Nurse Reviewer for the VA Surgical Quality Improvement Program (VASQIP): 12 months at $__________.__ per month = $_________.__. Line Item 0002: Option Period 1 (1 Sep 2012 - 31 Aug 2013): Services of Clinical Nurse Reviewer for the VA Surgical Quality Improvement Program (VASQIP): 12 months at $__________.__ per month = $_________.__. Line Item 0003: Option Period 2 (1 Sep 2013 - 31 Aug 2014): Services of Clinical Nurse Reviewer for the VA Surgical Quality Improvement Program (VASQIP): 12 months at $__________.__ per month = $_________.__. Line Item 0004: Option Period 3 (1 Sep 2014 - 31 Aug 2015): Services of Clinical Nurse Reviewer for the VA Surgical Quality Improvement Program (VASQIP): 12 months at $__________.__ per month = $_________.__. Line Item 0005: Option Period 3 (15 Aug 2015 - 14 Aug 2015): Services of Clinical Nurse Reviewer for the VA Surgical Quality Improvement Program (VASQIP): 12 months at $__________.__ per month = $_________.__. The resultant contract will be an Indefinite Delivery Contract as the exact number of cases to be reviewed cannot not be clearly identified. This will be a single award IDIQ and the total minimum quantity of this contract will be 30 cases for in-depth VASQIP review.. Non-Personal Services Contract The services provided in the contract will involve non-personal services. The personnel rendering the services of the contract are not subject, either by contract terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. Proposal Package The Offeror's quote shall contain the following: "1 copy of this posting with the Line Items filled out, and the Acknowledgement of the Amendments. "FAR 52.212-1 paragraph (b) Submission of Offers, Offers shall submit on letterhead stationery as follows: - The solicitation number; - The time specified in the solicitation for receipt of offers; - The name, address, and telephone number of the offeror; - A technical description of items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation (This may include product literature, or other documents). A requirement of this solicitation is that respondent has been trained and has experience as a Surgical Clinical Nurse Reviewer and must have demonstrated proficiency by passing the VASQIP SCNR Competency Module and the Annual VASQIP SCNR Competency Exam; - Any discount terms; - Remit to address if different than mailing address; - A completed copy of the representations and certifications at FAR 52.212-3 or 52.212-3(b) if representations and certifications are available through the Online Representations and Certifications (ORCA); - Past performance information to include recent and relevant contracts for the same or similar items and other references ( including contract numbers, points of contact with telephone numbers and other relevant information); and - Include a statement specifying the extent of agreement with all terms, conditions, and provisions included in this combined synopsis/solicitation. Offers that fail to furnish required representations or information or reject the terms and conditions of the solicitation may be excluded from consideration. "Offerors must be registered and in active status in Central Contractor Registration (www.bpn.gov/ccr/) and can complete their Online Representations and Certification at orca.bpn.gov.
