Drawings Statement of Work Market Survey Form - Interested Vendors Please return by 8/01/2011 Item Description QTY Unit Pink Boll Worm Traps 145,250 ea IAW the specifications First Article Approval-Government Testing will be required IAW FAR 52.209-4. The USDA Animal Plant Health Inspection Service, Plant Protection and Quarantine (PPQ) is considering the purchase of Pink Boll Worm Paper Delta Traps (PBWPDT) Offeror will submit a firm-fixed price offer, including transportation, FOB destination to: USDA APHIS IS, 3549 E. 14TH St., Ste FG, Brownsville, TX 78521 The traps will be used primarily for the surveillance and monitoring program associated with this pest. Background: The PBWPDT is utilized in monitoring of the Pink Boll Worm (Pectinophora gossypiella) a highly destructive insect of cotton. The goal of APHIS-PPQ is to define the extent of these infestations or potential infestations and limit artificial spread beyond the infested area through quarantine and an active regulatory and treatment program. These programs are an effective Federal-State-International partnership that prevents the establishment of Pink Boll Worm in new areas. In order to carry out this program's objective, PPQ requires Paper Delta Traps to conduct insect presence surveys to support decision-making for program activities. Objective: The Pink Boll Worm program utilizes this PBWPDT in conjunction with specific pheromones to effectively monitor populations. Without the requested traps, effectively defining infestations might not be possible and the ultimate cost of an eradication or control project could exceed millions of dollars should this trap not be available. PAGES 1 Survey Form (Interested Parties Please Return) 1 2 Statement of work 7 3 Drawings 3 First Article Procedures: The USDA may require first article testing in two phases after the award of the contract. Phase 1: After receiving approval/notification from CO or Contract Administrator (CA) of award the contractor will first submit twelve 8 inches by 8 inches paperboard samples, within 15 working days of notification of award, to: USDA APHIS PPQ Attention: Natalie Leva CPHST Otis Laboratory 1398 W. Truck Rd. Buzzards Bay, MA 02542 Testing will be conducted to determine if the standards have been met. Within 15 working days after the Government receives the first article, the CO or CA shall notify the contractor, in writing, of the conditional approval or disapproval for the submitted paper board. The notice of conditional approval or disapproval shall not relieve the Contractor from complying with all requirements in the specifications and all other terms and conditions of this contract. A notice of conditional approval or disapproval shall state any further action required of the Contractor. Phase 2: After successfully submitting the approved or conditionally approved paperboard samples that met the first article phase 1 testing the contractor shall then submit twelve (12) paper delta traps with adhesive, four of EACH COLOR, within 15 working days from the date of approval of the paperboard samples for the second phase of first article tests. Shipping documentation shall contain the government procurement number and the item identification. The characteristics that the first article must meet and the testing requirements are specified elsewhere in this contract. Samples should be submitted to: USDA APHIS PPQ Attention: Natalie Leva CPHST Otis Laboratory 1398 W. Truck Rd. Buzzards Bay, MA 02542 Within 15 working days after the Government receives the phase 2 first article, the CO or CA shall notify the Contractor, in writing, of the approval, conditional approval, or disapproval of the first article. The notice of conditional approval or approval shall not relieve the Contract from complying with all requirements for the specifications and all other terms and conditions of this contract. A notice of conditional approval shall state any further action required of the Contractor. A site visit may be arranged to the production facility to ensure the trap produced conforms to specification. Should any cases of production over-runs or slight dimensional changes caused by lot quantity procurement from cutting of base material be identified the CO or CA shall be made aware. Appropriate course of action will be directed after consideration of the Government needs and how these traps may fit them. Quality Assurance and Quality Assurance Surveillance: The contractor may be asked to provide a written copy of its quality control program utilized in the manufacturing process to ensure compliance with the specifications. The government may periodically test samples of the traps at its choosing to ensure they are compliant with the terms of the contract. The vendor shall provide five traps without adhesive for every five cartons of completed traps for each production to run. The traps shall be separately packaged and not included in the same cartons with the completed traps with adhesive. These traps will be used as a quality control of the amount of adhesive that goes on each individual trap. However, it is imperative that these traps are properly identified by purchase order number, production date and be of the same paperboard used in the production run. Separate packaging excludes these traps from any adhesive contamination. Weight comparisons will be made. This is not a request for competitive proposals, but instead a market research method to determine interested sources. A determination by the Government not to compete this requirement based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. No requests for capability briefings will be honored as a result of this notice.
