- GENERAL INSTRUCTIONS (a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code, 511210, and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is $25,000,000.00. Pre-quote discussions: The Pre-quote period is the period of time between issuance of the solicitation and due date for offers. This time period allows for an exchange of information between the VA and all potential offeror's. All requests to obtain a copy of the RFP and/or submit questions regarding this solicitation shall be sent in writing directly to the Contracting Officer by E-mail to dana.diamond@va.gov. The last day to submit questions shall be 29th July 2011, 6 p.m. Central Standard Time. All questions should reference the specific reference to the particular section/subsection number of the RFQ they are requesting be clarified. Questions must be received before the date specified for receipt of questions. Questions or requests for extension submitted after the cut-off date will not be considered Answers to questions and clarifications will be posted to FEDBIZOPPS. Any changes to this combined solicitation synopsis will be issued by amendment. The following information shall be submitted separately: "One original copy of Standard Form 1449 signature page (completing blocks 17a, 17b, 30a, 30b, and 30c.) submitted in hard copy with original signature. "Administrative information document including your official business name, physical street address, mailing address, phone number, email address of a primary and secondary point of contact. Additionally, the administrative information sheet shall include the offeror's DUNS number, cage code, NAICS, SIC, TIN and parent TIN. "Pricing Offer on an Excel Spreadsheet. All proposed pricing must be provided utilizing the CLINs and format provided in the RFP. The Offeror shall include all supporting details necessary for the Government to determine the Offerors price and ability to provided licensure, as well as the adequacy and advantages of the proposal. Veteran-Owned Small Business (VOSB) and Service-Disabled Veteran-Owned Small Business (SDVOSB) Status To receive credit under the VOSB and SDVOSB Status evaluation factor, an offeror must be registered at www.vetbiz.gov as a veteran-owned or service-disabled veteran-owned small business. Proposal submission instructions. All proposals shall be submitted electronically no later than 11th Aug 2011, 6 p.m. Central Standard Time. The Contractor shall submit their quotations to: E-Mail to: DANA.DIAMOND@VA.GOV Your entire quote must be received prior to the time and date specified above in order to be considered for award. Late or partial proposals will not be considered. The Government shall be evaluating quotations on the basis of the lowest technical cost and the written material presented. The vendor's quotation should be written in plain language (VA prohibits the use of elaborate technical quotation) and be formatted in Times New Roman 12 Pt Font. Department of Veterans Affairs reserves the right to conduct an on-site inspection of offered facilities. All responsible sources that are able to demonstrate their ability to meet the requirements and provide items as listed above may respond to this solicitation. Scope of Work: The Government requires an online enterprise-wide license to a Nursing Procedures and Skills competency assessment and database that can be utilized throughout the Veterans Health Administration. This online program is to be housed, maintained and managed by the vendor on the vendor's server. Background: Employee Education System (EES) delivers the highest quality products using a wide array of media to meet customer expectations. The traditional paper printing methods coupled with the technology advancements in distance learning are leveraged to create products in alternative formats to meet the diverse customer needs. As the VHA key source, EES assists customers in conceptual design, media selection, production scheduling, contract specification development, performance monitoring, and delivery through various modalities. VHA Medical facilities require an online system to allow them to record and maintain a database of standards for nursing procedures. In addition to this database, they also require the ability to test the competency of the nurse performing this procedure. The competency assessment can be used for new hires, for nurses switching specialty areas as well as ongoing competency validation. Each facility requires the ability to add, delete, and edit the procedures supplied in the product. In an effort to decrease duplication of contractual efforts, acquisition of an enterprise license for VHA Medical facilities is warranted. Objectives: a.The Nursing Procedure and Skill Program must include detailed descriptions of a complete set of Nursing Procedures allowing hospitals to record and maintain official standards for procedures. b.The Nursing Procedure and Skill Program database must be evidence based. c.The Nursing Procedure and Skill Program must be customizable allowing facilities to augment the stored information to meet specific facility level criteria. d.The Nursing Procedure and Skill Program must have the capability of restricting facility level customization to the identified facility level administrators. e.The Nursing Procedure and Skill Program must allow nurses to refresh their knowledge of how to perform a skill and should include a mechanism to measure individual competency. f. The Nursing Procedure and Skill Program must be interactive and provide resources for an array of healthcare settings to include hospitals, long-term care facilities, home health care, primary and acute care. g.The Nursing Procedure and Skill Program should contain a detailed description of the action to include an overview, equipment, equipment preparation, implementation, complications, special considerations and documentation. h.Multiple media formats should be utilized to relay the Media. i.Each procedure must be compliant with the latest procedural guidelines set forth by recognizable authorities including the Joint Commission and the American Associations of Critical Care Nurses.
