- The Contracting Department, NAVSUP Fleet Logistics Center Norfolk, VA, 23511-3392 intends to negotiate a sole source Indefinite Delivery/ Requirements contract with Firm Fixed Price provisions subject to FAR Part 12, Acquisition of Commercial Items in conjunction with FAR Part 15, Contracting by Negotiation with AMO USA, Inc., 1700 East St. Andrew Place, Santa Ana, CA 92705 for Naval Medical Center, 54 Lewis Minor Street, Portsmouth, VA 23708-2297 and Naval Hospital Camp Lejeune, 100 Brewster Blvd Camp Lejeune, NC 28547. The Contractor shall provide Two Treatment Cards, P/N 00303057-11 (50 EA), License Fees, P/N 1000-0100 (100 EA), Second Day Shipping (4 EA), High Myopic Custom Added Vue Pack, P/N 0030-4835 (300 EA), which includes Custom Added Vue License Fee (10 EA), Custom Added Vue Treatment Card (10 TX), Custom Added Vue Enhancement Card (2 TX), Prevue Card (2 TX), and Prevue Lens (2 EA), and an additional Second Day Shipping (4 EA) for Naval Medical Center Portsmouth. For Naval Hospital Camp Lejeune, the contractor shall provide two hundred (200) CustomVue Vision Key Treatment Cards and License fees package (part# 00304835) consisting of one card with 10 treatment each, fifty (50) each of Star S4 Lisik/PPK Card and License fee package (part # 0030-335-11), Calibration Cards (part # 0030-4016) and PI Interface kits for intralase laser. Each Treatment card package shall include ten (10ea) CustomVue License fees, (10Tx) CustomVue with High Myopic Treatment Cards, (2 Tx) CustomVue Enhancement Cards, (2 Tx) PreVue Cards, (2ea) PreVue Lens. These cards will be used with Government owned STAR S4 IR Excimer Laser Correction System manufactured by AMO USA, INC. A complete RFQ package will be available on or about 20 July 2011 by downloading the documents at the Neco Website as follows: http:// www.neco.navy.mil. The North American Industry Classification System (NAICS) code for this acquisition is 339115. Potential contractors are hereby notified that the solicitation and subsequent amendments to the solicitation will ONLY be available for download at the above site; this office will no longer issue hard copy solicitations. By submitting an offer, the vendor is self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any federal agency. Failure to do so may represent grounds for refusing to accept the offer. By submission of an offer, the contractor acknowledges the requirement that a prospective awardee must be registered in the CCR database prior to award, during performance, and through final payment of any contract resulting from this solicitation, except for awards to foreign vendors for work to be performed outside the United States. Lack of registration in the CCR database will make a contractor ineligible for award.
