Description

ATT6 PAST PERFORMANCE QUESTIONNAIRE - MUST BE RETURNED BY REFERENCE ONLY ATT5 EXP QUESTIONNAIRE ATT4 DRAWINGS ATT3 SPECS ATT2 WAGES ATT1 BID METHOD B ATT1 BID METHOD A RFP PKG RFP FORM Project includes asphalt surface on National Forests System Road #711 Wine springs - White Oak Road on the Nantahala Ranger District. Work consist of removal and prep of existing surface and prep for new overlay. Also included is pavement marking of the asphalt sections replaced. Description of methods is as follows: Method A - Contractor shall remove existing asphalt areas as identified within the project description by milling and incorporating material back into roadway. Surface shall meet project specifications by blading, compacting, shaping prior to approval to overlay of super pave 2" depth - S9.5B. Method B - Contractor shall prep existing asphalt sections by blading areas that are unsuitable, pulverizing existing asphalt and incorporating into the base material. Contractor shall overlay with 2" depth -l19B and then a surface course of 1" depth -S9.5B as a running course. Solicitation package will be uploaded to FBO on July 16, 2011.
