Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-11-Q-0010 and is being issued as a Request for Quote (RFQ). This procurement is 100% Set-Aside for Small Business. The associated NAICS code is 541614 with a size standard of $7.0M. All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the offeror's responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation to include all charges for the following items to be delivered in accordance with the attached Performance Work Statement (PWS) and information below: 0001- Material Handler & Forklift Services @ USACE Finance Center located at Naval Support Activity, Millington, TN. The Contract will be for one (1) base year with four (4) option year periods. Quotes shall include pricing for base year and all four option years. Partial quotes will not be accepted. (A Quote form is included as Attachment 1, however quotes may be submitted on company letterhead) The Contractor shall furnish all supplies and services necessary to provide Material Handler and Forklift Services including but not limited to: The movement of supplies and materials and establishment of proper warehousing plans to facilitate storage and retrieval, considering shape, quantity, use, and possibility of damage, deterioration and contamination to or from other items. Assigns locations for bulk as well as commonly-used items. Takes periodic inventories of stock levels and initiates action to replenish. Properly disposes of recyclable items such as paper, plastic, cans, cardboard, and excess pallets. Delivers supplies to requestor and completes appropriate paperwork. Forwards paperwork to individual responsible for final accounting. Receives incoming items from trucks or mail. Assists in unloading of trucks as required, by using equipment such as forklifts, hand trucks, pallets, jacks, or dollies. Moves furniture upon request. Documents receipt of material by checking received items against vendor's documents and government purchase orders or requisition documents and forwards to appropriate individuals. If condition, quality and quantity do not match, refuses shipment, makes proper annotation, and refers to appropriate individual. Assembles equipment received as needed. May use items such as hammers, pliers, strapping, and stretchwrapping tools and equipment, tape fasteners, metal band shears, and related equipment. Delivers to user upon completion. Sets-up and breaks down chairs and/or tables for training sessions, town hall meetings, etc. Operates electrically powered forklift capable of lifting 2000-6000 pounds as high as 168 inches to load, off-load, stock, unstock, warehouse, re-warehouse, and move boxes, crates, cartons, and pallets. Regulates forklift movements, exercising care to avoid overloading or improper stacking or movement of materials. Performs operator maintenance in accordance with rules and regulations. The following skills are required: Knowledge of overall warehousing plans and accepted warehousing methods, procedures and techniques to select proper storage locations and store, stock, and palletize items. Skill in operating a forklift within the narrow confines of a warehouse area and in stacking items in accordance with size, weight, shape, and quantity of material. Skill in use of manual dollies, electric jacks and hand tools used in warehouse tasks such as hammers, pliers, strapping, and stretch wrapping tools and equipment, tape fasteners, metal band shears, and related equipment. Skill in assembly of equipment, e.g., task chairs, tables, etc. Ability to lift/carry items that may weigh up to 70 pounds, some with assistance of weight handling equipment. Often required to stand on hard surfaces for extended periods of time, and bend, stoop, and work in tiring and sometimes uncomfortable positions.
