- Technical Surveyor Consultation SOW TECHNICAL SURVEYOR CONSULTATION FOR THE ESRD PROGRAM SURVEY & CERTFICATION, DURING THE ONGOING IMPLEMENTATION OF NEW ESRD REGULATIONS Solicitation# 2211308601 This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; A SEPARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The Centers for Medicare and Medicaid Services intends to award using Simplified Acquisition Procedures, a firm fixed priced purchase order for "Technical Surveyor Consultation for the End Renal Disease (ESRD) Program, Survey & Certification, during the Ongoing Implementation of New ESRD Regulations. The purpose of this request is to purchase services from a contractor who has technical experience as a surveyor in dialysis facilities to assist in the ongoing implementation of the ESRD Program under new regulations. This contract will include assistance with the ongoing analysis and interpretation of the new ESRD conditions; assistance with the development and implementation of ESRD survey protocols and tools; development of ESRD training programs and associated multimedia training tools; consultation on surveyor issues related to clinical practices in dialysis facilities; and assistance with updating and teaching a revised automated survey system, i.e., the STAR program. The period of performance will be 1 year. The North American Industry Classification System Code (NAICS) is 541618. The Statement of Work/Deliverables and Technical Evaluation Criteria are attached. This Request for Quote (RFQ) is being set aside for small business concerns. Responses must be sent electronically in MS Word format to Lucille.Lee@cms.hhs.gov and Louis.Anderson@cms.hhs.gov no later than 11:00am on July 27, 2011. Responses shall be no more than ten pages and shall include the following: 1) Soundness of Approach; 2) Personnel; 3) Past performance information - relevant experience to perform the required services; 4) Management and 5) Price. All responses will be evaluated on a best value basis. The government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price and other factors). All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict or the appearance of a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. Please submit proposals in whole dollar amounts only.
