Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.106, as supplemented with additional information included in this notice. Mission and Installation Contract Command (MICC) Fort Belvoir, VA is releasing this combined synopsis/solicitation electronically only. This on-line version of the Request for Quote (RFQ) announcement constitutes the only solicitation; a written solicitation will not be issued. The Defense Acquisition University (DAU) respectfully requests to host a one day Health Fair between: July - September 2011, from 10:00 a.m. - 2:00 p.m. at the Fort Belvoir, VA DAU campus for DAU employees. Trained and qualified personnel are required to administer and conduct all medical testing procedures. The exact date of the Health Fair will be coordinated by the selected contractor and DAU personnel after contract award. Attendance is estimated at 100-150 employees. The Mission and Installation Contracting Command, Fort Belvoir, Virginia intends to award a Firm Fixed Price contract based on Lowest Price Technically Acceptable (LPTA) Methodology. Resultant contract will be awarded to the offeror whose quote conforms to this combined Synopsis/Solicitation, statement of work (SOW) documentation, is deemed acceptable, and provides the lowest price. The SOW will be uploaded under a separate attachment. Offerors shall specifically indicate the medical certifications and or registrations held which shows the ability to administer and conduct all testing procedures that are requested. The certifications and or registrations must be issued by a state agency or educational institution. Resumes are not required to be submitted. Offerors shall provide pricing in accordance with the statement of work. This procurement is 100% set-a-side for Small Business.
