- This is a Pre-Solicitation Notice for MRI Services being released for a commercial item prepared in accordance with the Federal Acquisition Regulation (FAR) Part 5.2. This Pre-solicitation Notice is issued to inform prospective Contractors of the intent of the Department of Veterans Affairs, VISN 4 Contracting Activity to negotiate a Health Care Resources commercial contract under the authority of FAR Part 12, Public Law 104-262, VAAR Part 873 and 38 U.S.C. 8153 to provide Magnetic Resonance Imaging (MRI) for Erie VA Medical Center patient at the Contractor's location. The information provided in this Pre-solicitation Notice is subject to change and is not binding on the Department of Veterans Affairs. If a Request For Proposal (RFP) is released, Proposals for the MRI services will be received at VISN 4 Contracting Activity, Department of Veterans Affairs, 135 East 38th St., Erie, PA 16504 by mail or by hand delivery by 12:00 PM, local time on July 2, 2011. Contractors are advised that the United States (U.S) Government will not pay for any information or administrative cost incurred in response to this Pre-Solicitation Notice or an RFP. All costs associated with responding to an RFP will be solely at the responding party's expense. Please be advised that all submissions become Government property and will not be returned. Offerors are advised that it is your responsibility to review and monitor the Federal Business Opportunities (FEDBIZOPPS) www.fbo.gov website frequently to ensure you have the most up to date information that may pertain to this Pre-Solicitation Notice or a future RFP. Personnel assigned by the Contractor to perform the services covered by this contract shall be a citizen of the United States. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine is required. Personnel must have a current, full and unrestricted license to practice medicine in a State, Territory, or Commonwealth of the United States or the District of Columbia, and meet the physical standards for the position. Physicians appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C 7402(d). The qualifications of such personnel shall also be subject for review by the Chief of Staff. It is preferred that the designated physician(s) possess board certification by the American Board of Medical Specialties (ABMS), or the Bureau of Osteopathic Specialties, or board eligible within 4 years of residency or fellowship completion. The physician's qualifications will be subject to the credentialing and privileging process by the Erie VA Medical Center (EVAMC) and subject to Erie VAMC's requirement for VetPro. The Contractor will provide resumes, licensures/certifications, and/or proof of competency of all personnel who will be performing services under the contract. Records will be submitted to the EVAMC with the Offeror's Proposal. Contractor will also be responsible for providing their physician's National Provider Identifier numbers (NPI). The NAICS Code is 621512. All Offerors must be registered in CCR database, see http://ccr.gov, D&B at www.dnb.com and Online Representations and Certifications must be completed at https://orca.bpn.gov. To be recognized as a Veteran owned business, registration is required in VETS at www.dol.gov/vets and Vet Biz at www.vetbiz.gov. Ensure your registration on each site is correct, complete and has not expired. All questions must be submitted in writing by mail, fax or email and must contain a point of contact, the RFP name and number, phone and fax numbers, and must be addressed to the Contracting Officer. The questions should be listed in numerical order and must be received by the Contracting Officer five (5) or more business days prior to the Proposal deadline. Telephone calls will not be accepted.
