- VA-69D-11-RQ-1247 The Great Lakes Acquisition Center (GLAC) in Milwaukee, WI intends to negotiate on a sole source basis (IAW FAR 13) with Optos North America of Marlborough, MA as the only distribution source that provides The Ultra wide Field Flouresein Angiography System used for patient care. Optos North America is the only company that makes a system that meets our requirement at Hines. Specifically, the contractor shall provide the following: - Ultra wide Field Flouresein Angiography System with all associated equipment and supplies (Part #P200MA, Quantity 1); This acquisition is being conducted under simplified acquisition procedures (IAW FAR 13). There are no set-aside restrictions for this requirement. The intended procurement will be classified under North American Industry Classification System (NAICS) 33912 with a Small Business Size Standard of 500 employees. This notice of intent is not a request for competitive proposals and no solicitation document exists for this requirement. However, parties interested in responding to this notice will need to submit technical data sufficient to determine capability in providing the same product. All capability statements received within 5 days after publication of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. Capability statements shall be submitted by email only as a MS Word or Adobe PDF attachment to Scott Ivy at scott.ivy@va.gov Statements are due by 12:00 CST 22 July 2011. No phone calls accepted. JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION Sole Source for an OPMI Lumera 700, for Hines VA VA-69D-11-RQ-1036 DEPARTMENT OF VETERANS AFFAIRS Justification and Approval For Other Than Full and Open Competition 1.Contracting Activity: Department of Veterans Affairs, VISN 12, Great Lakes Acquisition Center, 5000 W. National Avenue, Milwaukee, WI 53295. 2. Nature and/or Description of the Action Being Processed: Justification and approval for Other Than Full and Open Competition for the purchase of one (1) Optos P200MA ultra-wide field fluorescein angiography instrument for the Edward Hines VA Hospital in Hines, IL per Purchase Request# 578-11-3-969-0151. Award of a new firm-fixed price contract is anticipated. This procurement will proceed on a sole source basis with Optos North America. 3. Description of Supplies/Services Required to Meet the Agency's Needs: The Edward Hines VA Hospital has a requirement for one (1) Optos P200MA ultra-wide field fluorescein angiography instrument, stock #P200MA. The total for the system is $150,000.00. Delivery is expected within 90 days of the Notice of Award. 4. Statutory Authority Permitting Other than Full and Open Competition: FAR 6.302-1(a)(2). The VA's minimum needs can only be satisfied by unique supplies or services available from only one source (41 USC 253 (c)(1)). 5. Demonstration that the Contractor's Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): The Optos P200MA ultra-wide field fluorescein angiography is the only product that has the following unique features: produces ultra-widefield,(200 degrees) dynamic high-resolution angiographic images of the retina. Optos' technology uses low-powered red and green lasers to scan approximately a 200 ° interior angle of the retina and converts the reflected light into the digital optomap image. The addition of a blue, 488nm laser to this platform allows ophthalmologists to perform fluorescein angiography (FA) of the same ultra-widefield view, creating a stack of sequential images called optomap fa. Additional features of the P200MA system include: An image recognition system that facilitates patient positioning, DICOM compatibility, Patient gaze control, All Optos instruments are designed to image through a minimum of a 2-mm aperture and therefore, the decision to dilate the patient can be made by the physician. Viewing out to the edges of the retina greatly increases our chances of catching capillary drop-out and neovascularization that previously went unseen in diabetic patients. The ability to take pictures quickly with a panoramic view, zoom in to a point of interest and capture important findings using Optos' proprietary screening software program will be advantageous. It allows 45% more retina to be imaged. The ability to better visualize areas of peripheral ischemia will likely lead to earlier identification of eyes at higher risk of disease progression and earlier intervention. Also, this will enable serial photo comparison of choroidal nevi and tumors in the peripheral retina. 6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: Other vendor's products were evaluated but did not have all the required features. 7. Determination by the Contracting Officer that the Anticipated Cost to the Government will be Fair and Reasonable: Negotiation will be made at time of award prices. 8. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Dr. Pearlman reviewed the different products available through literature, conversations with colleagues and vendors at the American Academy of Ophthalmology annual meeting. The contracting officer searched the SBA, GSA, VA FSSand internet. This camera system is the only stereo camera for the slit lamp that was found that captures ultra-widefield,(200 degrees) dynamic high-resolution angiographic images of the retina. There was no comparable brand able to perform to the VA requirements. 9. Any Other Facts Supporting the Use of Other than Full and Open Competition: The optomap fa dynamic ultra-widefield angiographic study is a more valuable approach than montaged narrow field-of-view images that only provide a composite from different parts of the retina taken at different points during the circulation of fluorescein. Also, the capability to image through small pupil is an enormous advantage. We intend to award a sole source award to Optos North America for the purchase of an Optos P200MA ultra-wide field fluorescein angiography instrument 10. A Statement of the Actions, if any, the Agency May Take to Remove or Overcome any Barriers to Competition before Making subsequent acquisitions for the supplies or services required: None. 12.
