- SOLICITATION NOTICE VA-69D-11-RP-0461 PET/CT SCAN SERVICES The GLAC (Great Lakes Acquisition Center) is soliciting proposals for PET/CT Scan Services for beneficiaries of the Department of Veterans Affairs Medical Centers (VAMC) at The Edward Hines Jr. VAMC, 5000 S. 5th Avenue, Hines, IL 60141 VA Hospital from September 1, 2011 thru August 31, 2012 with up to four one-year options to extend. All qualified offerors/firms who believe they can satisfy the solicitation requirements are invited and encouraged to provide their proposal. Offers shall be evaluated for fair and reasonable pricing, technical competency/capability to perform services, Socioeconomic consideration and satisfactory past performance record. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. Contractor to provide FDA Approved Positron Emission Tomography (PET)/Computed Tomography (CT) Scan Services for beneficiaries of the Hines VA. ALL SERVICES SHALL BE PROVIDED AT THE CONTRACTOR ™S FACILITY AND MUST BE A FIXED SITE. Contractor will provide labor, materials, supplies, equipment, certified personnel, and radioactive tracers and incidentals required to perform the scans. Contractor shall provide overall supervision of exams. DESCRIPTION OF FACILITIES: Contractor ™s site must be within a 15 mile radius of The Edward Hines Jr. VAMC, 5000 S. 5th Avenue, Hines, IL 60141. Adequate parking should be available for patients at minimal or no cost to the patient. The facility will maintain comfortable and clean facilities and appropriate seating area while patient is waiting for their scan. Contractor ™s facilities must be accessible for wheelchair and stretcher, as well as ambulatory patients. The Contractor shall take full responsibility for the patient ™s well-being and safety once the patient enters its facility. This is a Firm Fixed Price contract and this acquisition is unrestricted, Full & Open Competition. The size standard for North American Industry Classification System (NAICS) code 621512 is $13.5M. The Solicitation will be available at fbo.gov and posted under the same link as the Solicitation Notice. All amendments will be posted on fbo.gov and it is the responsibility of the contractor to monitor for amendments. The Solicitation is anticipated to be released on or about July 13, 2011 with an anticipated proposal due date of July 27, 2011 (Dates subject to change).
