Solicitation Number: Notice Type: VA-258-10-RP-0222Combined Synopsis/Solicitation Synopsis: (i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. (ii)Reference attached Solicitation VA-258-10-RP-0222 is being issued as a Request for Proposal. (iii)The attached solicitation (SF 1449) document and incorporated provisions and clauses for commercial items are those in effect through Federal Acquisition Circular 2005-53. (iv)This is a 100% small business set-aside. The North American Industry Classification System (NAICS) code is 621210 and the business size standard is $7.0 Million. (v)Contract line item numbers used are Dental Services ADA Codes, unit is 1 each. The term of the agreement is for a base year, with four (4) one-year option periods. (vi)The contractor shall provide Dental Services for VAMC Phoenix patients at the contractor's place of business to include but not limited to all management, supervision, manpower, materials, supplies and equipment in accordance with the requirements of this agreement. (vii)Place of delivery is Contractor's place of business. (viii)Provision at 52.212-1, Instructions to Offerors - Commercial and Addendum to 52.212-1 Instructions To Offerors--Commercial Items, applies to this acquisition. (ix)Provision at 52.212-2, Evaluation -- Commercial Items applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical and past performance, when combined, are EQUAL TO PRICE. (x)Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (xi)Clause at 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii)Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. (xiii)Reference attached Solicitation VA-258-10-RP-0222 for additional contract requirement(s) or terms and conditions (such as contract financing arrangements or warranty requirements) determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices. (xiv)Proposals shall be submitted electronically via the Department of Veterans Affairs (VA) Vendor Portal (https://www.vendorportal.ecms.va.gov ) by August 12, 2011 at 10AM local time (Arizona). All potential offerors are required to register and have a completed registration in the Central Contractor Registration (CCR) in order to submit proposals to the VA Vendor Portal web site. Electronic proposal submissions are to be submitted on the VA Vendor Portal saved in Microsoft Word or Adobe Acrobat format and prices using Excel spreadsheet. ("Read Only" files are acceptable). All electronic files shall be virus checked prior to submission. Additional information concerning the use of the VA Vendor Portal can be found in ATTACHMENT G - eCMS_ Vendors_Guide_v4_0. If an offeror encounters difficulty submitting their proposal on the VA Vendor Portal screen shots of the issue shall be taken and submitted with the proposal. The alternative electronic delivery of the proposal shall be to Carolina.mcfaul@va.gov. This alternative shall only be used if an offeror is unable to submit their proposal using the VA Vendor Portal. (xv)Any explanation desired by an offeror regarding the meaning or interpretation of the solicitation must be requested in writing through the VA Vendor Portal by July 18, 2011 3:00 pm local time (Arizona) where no further questions will be accepted to allow sufficient time for all offerors to review the response provided before the submission of their offer. Responses to questions will be posted as an amendment to the solicitation and the government reserves the right to consolidate questions and responses.
