- This is not a solicitation announcement or a Request for Quotations. The purpose of this RFI is to gain knowledge of potential qualified sources. Responses to the notice shall be e-mailed to jonathan.jewel@va.gov. Please note that responses that identify errors, deficiencies, or areas of improvement in the following description of tasks may be used to improve any future RFQ for these services. Please refrain from submitting unique or proprietary information. (see note below) Scope The purpose is to provide vehicle services for beneficiaries of the Department of Veterans Affairs Medical Centers (VAMC) in Portland and the Vancouver Washington, and the Hillsboro Community Based Outreach Clinic (CBOC). The awarded Contractor shall provide all vehicles, drivers, dispatchers, mobility managers, management, supplies, maintenance, fuel, and equipment necessary to provide services, as identified on the Schedule. Objective The contractor will provide a shuttle service from the beneficiary's residence or requested location pick-up point to his or her appointment at the VA Medical Center or CBOC. This service will include a return trip, if requested or required. The Contractor is expected to take Veterans calls requesting transportation to a VA appointment at any of the three designated locations. (In order for Veterans to qualify they must have a valid VA appointment and have requested transportation no later than the day before the transportation is required. The Veterans must be located for pick-up within the city limits of the destination location requested.) Locations: Portland Division3710 S.W. U.S. Veterans Hospital Road, Portland, OR 97239 Vancouver Division4th Plain & St. Johns, Vancouver WA 98662 Hillsboro CBOC1925 Amber Glen Parkway Ste. #300, Hillsboro, OR 97006 Operating Hours: 1.The contractor will be on duty Monday through Friday from 7:00AM - 5:00PM excluding federal Holidays. 2.Federal Holidays: are those holidays observed by the United States Government: i.e., New Year's Day, Martin Luther King Day, President's Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day and any other day specifically declared by the President of the United States to be a Federal Holiday. Scheduling and Routing: Interested firms shall provide the following: 1.Please submit a general capabilities statement on how your firm could adequately perform all the elements of the services described above. Please limit the capabilities statement to 5 pages although longer responses will be accepted provided they are concise and do not contain irrelevant information. 2. List no more than 3 (three) projects/contracts that your company has performed (as a prime vendor) of similar scope and complexity. Please be specific and provide complete reference information to include at a minimum the project name and the project's owner contact information. 3.Please provide a cover page with your firm's name and contact information. Please include your email, physical address and phone number. Please include your firms DUNS Number on the cover page. DUNS numbers can be obtained through http://fedgov.dnb.com/webform. 4.Industry Feedback (optional): Respondents may include suggestions or comments on the above description of services to help the Department of Veterans Affairs to improve subsequent solicitations for similar services. Please see note below. 5.Please submit responses to jonathan.jewel@va.gov no later than July 14, 2011. Late responses will be accepted but will not be used to determine potential set-asides.
