Description
Travis Air Force Base intends to solicit and award a contract for Surgical Instrument Maintenance and Repair Services as defined in the Performance Work Statement (PWS), and as specified in all Appendices to include but shall not be limited to inspection, maintenance and repair services of surgical instruments. Contractor will be responsible for servicing a large assortment of surgical instruments extending from but not limited to Minimally Invasive Surgery (MIS) to micro instruments that are used in (but not limited to) the following clinics and operating rooms; Otolaryngology, Laparoscopic, Plastic Surgery, Ophthalmology, Orthopedics, OB/GYN, Oral Maxillofacial, General, Urology, Vascular and Cardiothoracic at David Grant Medical Center, Travis AFB. Required services includes but are not limited to: inspection, maintenance, repair, cleaning, and sharpening of surgical instruments performed in accordance with manufacturer requirements. Provide loaner equipment within stated timeframes, off-site maintenance services, repair obsolete equipment to maximize the life expectancy of equipment, test electrosurgical insulation of scopes/endoscopic instrumentation and repair any items that do not meet minimal manufacture and safety requirements, provide diamond dusting of microsurgical instrumentation, air hose repair, Castroviejo needle holder, specialty locks an springs replace, laparoscopic rebuilds, continuity testing of bipolar instrumentation and cords, and specific power equipment (Jacobs chucks, orthopedic attachments and bur guards), and inspections of back up instruments. This solicitation is full and open competition. We intend to award a contract with a base year plus four option years. Anticipated start date is 01 Oct 11, subject to the availability of funds. The North American Industry Classification System (NAICS) code is 811310 and the small business size standard is $7,000,000.00. The Federal Supply Class (FSC) is J066. The Standard Industrial Classification (SIC) is 7699. This Request for Proposal (RFP) #FA4427-11-R-0005 will be available, on or about 15 July 2011. All responsive offers will be considered by the agency. It is incumbent upon the interested parties to review this site frequently for any updates/amendments to any and all documents. Past Performance information will be required to be submitted by the date specified in the solicitation. A small business subcontracting is plan is required IAW FAR 19.702(a) (1) and (2) if you are a large business. Firms that are not responsive to the solicitation requirement will be ineligible for award. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). Per AFFARS 5352.201-9101: An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer, Ms Cora L. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
