Description

The Department of Veterans Affairs Medical Center, Hampton, Virginia (VAMC Hampton) is seeking potential sources to provide the facility, staffing, equipment and supplies /services required to provide Healthcare for Homeless Temporary Rehabilitative Residential Housing to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs Medical Center (VAMC) in accordance with the Performance Work Statement in the solicitation package. The Contractor shall provide housing to include, but not limited to beds, restrooms, bath, kitchen facilities, laundry facilities and related furnishings. Contractor shall also provide food daily for three balanced meals to include a daily snack as well as personal hygiene items, first aid supplies and storage as needed, transportation to and from the VAMC and other appointments as required. Other services required by the Contractor includes but are not limited to: wound care, ADL assistance, daily plan of care, structured group activities to include physical activities, monitoring of medications, supportive social services and basic case management services and assistance in learning and development of responsible living patterns. All Contractor staff members are required to be trained and competent in administering breathalyzer tests. It is understood that the type of beneficiaries to be cared for under this contract will normally require care and services over and above the level of room and board. The number of beds required is four. Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. This will be an Indefinite Delivery, Indefinite Quantity (IDIQ) type contract and this acquisition is unrestricted. The size standard for North American Industry Classification System (NAICS) code is 624221. The size standard is $7.0 Million. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: Name and address of company; business size (large or small); identify whether or not firm falls into any of the socioeconomic categories, and any pertinent information which demonstrates firms ™ ability to meet the above requirement.
