This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Property and Fiscal Office for Arizona intends to award a sole source fixed-price purchase order/contract to LivingWorks LP. to provide for a 5-Day ASIST Training Course. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-52, Effective 31 May 2011. This solicitation is being conducted on a sole source basis. Justification follows: SOLE SOURCE Justification (For Other Than Full and Open Competition) Reference: FAR 6.303-2 Section/Unit Name: JFHQ - G1 1.State Relevance Of Purchase To Your Mission, Purpose, Research or Study: The past few years have shown a disproportionate and growing number of combat and operational stress injuries, stress illnesses, and suicides with National Guard Service Members and their Families. Stress illnesses and injuries range from a Service Member having a significantly decreased work performance or marital conflict to PTSD, Depression, Anxiety and Substance Abuse. The most serious stress illness is suicidality. Reports for CY 2010 show 5 suicides, and 5 attempted suicides. Of note, an additional 3 deaths in CY 2010 are still under investigation, with the distinct possibility that the total for 2010 could reach an unprecedented 8 suicides. This unparalleled increase from previous years of recorded suicides is a lagging indication of the challenges Service Members and their Families face. Stress illnesses and injuries negatively impact mission readiness, performance and retention. The success of the Arizona National Guard requires a coordinated plan to build the Resiliency, Continuum of Care, Stress First Aid, and Suicide Prevention Training of AZNG Service Members and Families. Therefore, the Applied Suicide Intervention Skills Training (ASIST) program is highly needed in the AZ ARNG, and has been approved by the Army G-1. 2.Identify Supplies Or Services To Be Approved For Sole Source/Name Brand Treatment: The AZ ARNG is in need of purchasing the copyrighted ASIST program that was developed in Canada by the contractor for delivery of suicide intervention training. The purchased program contains: Trainers Manual, Participant Workbooks, Suicide Intervention Handbook, one Canadian produced video (60 Minutes), Suicide Awareness manual and other program materials. The Contractor will provide a Certificate of Qualification to each trainee upon the conclusion of the T4T training course. Provide details in Paragraph 9) A. [ ] Replacement Equipment - The purchase is for equipment associated with use of existing equipment where compatibility is essential for integrity of results.. Note: If item is available from more than one supplier, the item can be treated as proprietary, but must be solicited from multiple sources of supply. 9.Narrative Justification LivingWorks Education is the sole source in North America for providing the training materials for training materials for trainers and the participant materials used in the ASIST workshop per ALARACT 253/2009 - SUICIDE INTERVENTION SKILLS TRAINING. The associated NAICS code is 611430. The small business size standard is $7.0 million. Statement of Work MONDAY, August 29, 2011 - FRIDAY, September 2, 2011 Training for Trainers (T4T) in Applied Suicide Intervention Skills Training (ASIST) Phoenix, Arizona The Arizona National Guard is in need of a training for trainers (T4T) in Applied Suicide Intervention Skills Training (ASIST) five day course to be conducted at the Papago Park Military Reservation Monday, August 29, 2011 through Friday, September 2, 2011. The AZ ARNG will purchase the copyrighted ASIST program that was developed in Canada by the Contractor for delivery of Suicide Intervention Training. The purpose of this contract is to provide training and consultation in connection with the purchased program that will prepare candidate trainers with the skill and abilities to implement the purchased program. The following statement of work will outline exactly what is needed to perform the T4T ASIST training. The potential service provider will hereinafter be referred to as the "Contractor" within this statement of work. The proposal shall include a detailed description, of how potential suppliers intend to meet the statement of work requirements. ASIST T4T Training: The Contractor will train 15 Soldiers to be qualified ASIST Trainers at the Papago Park Military Reservation, 215th RTI, from Monday, August 29, 2011 through Friday, September 2, 2011. The Contractor will provide all training materials and supplies necessary for the 15 trainees to include: Trainers Manual, Participant Workbooks, Suicide Intervention Handbook, 1 Canadian Produced DVD Video (60 minutes), Suicide Awareness Manual, and any other program materials. The Contractor will provide a Certificate of Qualification to each trainee upon the conclusion of the T4T training course. The Contractor shall supply ASIST senior and coaching trainers to present ASIST skill training; 2 trainers for Monday to Tuesday and 1 additional trainer for Wednesday through Friday. Cost will be inclusive of ALL Taxes and Service Charges Contractor Expenses, Lodging, Travel and Post Security Considerations: The Contractor will arrange and pay for lodging accommodations to all Contractor trainers. The Contractor will provide transportation to all Contractor trainers. The Contractor will provide per diem to all Contractor trainers. All Contractors will present official government identification (ie. driver's license, passport, military ID) at the front gate entrance of Bushmaster Road on Papago Park Military Reservation, Phoenix, AZ. The following factors shall be used to evaluate offers: (i) technical capability of the item offered to meet the Government requirement; (ii) price; Technical capability will be rated on a pass/fail basis and will be evaluated on an offeror's ability to provide all required items identified in the Statement of Work. Technical capability is approximately equal to price. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with your offer.
