Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is W91YU0-11-T-0216 0010060593 and is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51. The associated North American Industrial Classification System (NAICS) code for this procurement is 621511 (Medical Laboratories), Size standard is $12.5 This RFQ is being issued as Firm Fixed Price, UNRESTRICTED. All responsible sources may submit a quotation which shall be considered by the agency. Non-Personal Service for Genetic Lab Testing for Breast and Ovarian Cancer located at William Beaumont Army Medical Center, El Pasco, TX, FOB Destination. These genetic lab testing services are for use at William Beaumont Army Medical Center, El Paso, TX and the proposed contract is for a base period of one year plus option periods. All Contractors shall provide an offer for the following CLINs and option line items, CLIN 0001, The contractor shall provide a Non-Personal Service for Genetic Lab Testing for Breast and Ovarian Cancer for patients that have been diagnosed with a high risk of hereditary Breast and Ovarian cancer (Comprehensive BRAC Analysis). Contractor must provide testing of the full sequence and large rearrangement analysis of BRAC Analysis 1 and BRAC Analysis 2 genes. The Contractor must provide testing for patients with hereditary non polyposis colorectal cancer (Comprehensive colaris), test must provide a full sequence and large rearrangement analysis of the MLH1 and MSH2 genes. Period of Performance: 1 Oct 2011 thru 30 Sep 2012 (Subject to the Availability of FY12 funding.) OPTION CLIN 1001: The contractor shall provide a Non-Personal Service for Genetic Lab Testing for patients that had been diagnosed with a high risk of hereditary Breast and Ovarian cancer (Comprehensive BRAC Analysis). Contractor must provide testing of the full sequence and large rearrangement analysis of BRAC Analysis 1 and BRAC Analysis 2 genes. The Contractor must provide testing for patients with hereditary non polyposis colorectal cancer (Comprehensive colaris), test must provide a full sequence and large rearrangement analysis of the MLH1 and MSH2 genes. Contractor must provide testing of the full sequence and large rearrangement analysis of BRAC Analysis 1 and BRAC Analysis 2 genes. Period of Performance: 1 Oct 2015 thru 30 Sep 2016 Pricing quote shall include all applicable fees.
