- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-11-Q-0016 and is being issued to solicit interest for the establishment of a Blanket Purchase Agreement (BPA). An official copy of the solicitation can be viewed by logging onto www.acquisition.army.mil.asfi/gov. This solicitation incorporates Wage Determination No. 2005-2495 Rev (12). This acquisition is an UNRESTRICTED procurement. The associated NAICS code is 621111. Quotes shall include pricing for (1) base year and (4) four option years. Establishment of a BPA does not imply any agreement by the Government to place future contracts or orders with the BPA holder. The Government makes no representation as to the number of calls or actual amount of work to be ordered. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. All responsible sources may submit a quote, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the offerors responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit an itemized/detailed quote on company letterhead to include all charges for each specified physical listed in scope of work. Response to this solicitation must be received via email, fax, mail, or delivery by June 24, 2011 no later than 10:00 PM CST at 167 North Main Street B202, Memphis, TN 38103. Vendors not registered in Central Contractor Registration (CCR) database prior to award will not be eligible for award. Vendors may register for CCR by calling 1-800-334-3414 or register online at www.ccr.gov Contractor shall submit quote for requirements listed below: SCOPE OF WORK 1.DESCRIPTION OF SERVICE Contractor shall provide non-personal physician services for employees at the Memphis District, Ensley Engineering Yard (EEY). Services consist mainly of but not limited to conducting medical surveillance physicals. These physicals include OSHA mandated physicals (29 CFR 1910 and 1926), USCG license physicals (Navigation and vessel inspection Circular No.02-98), and general occupational physicals. Examinations will be conducted per guidelines in MDR 385-1-14, regulatory guidance, and the examining physician's discretion. Contractor shall provide all necessary labor, equipment, material, supervision, and services to perform work in accordance with this scope of work. 2.PERFORMANCE REQUIREMENTS All physicals will be performed in Memphis, TN. Physicals will be one of three types: General Physical, Diver Physical, and Welder Physical Each employee will be provided a physical checklist indicating service to be performed by contractor. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the Medical professional aspects of services rendered (e.g., professional judgments or diagnosis of specific medical findings). 95% of all medical charts are to be updated and returned to the EEY Safety Officer within 10 workdays and 100% of the charts within 15 workdays of the physical inception. Once a schedule is developed, the Chief, Safety Office, and EEY Health Clinic RN shall provide the contractor with changes, for example, vacation, continuing education, with a 2-week notice. The employees shall report punctually to the Contractor's site for scheduled physicals. (When situations arise that will delay reporting by 30 minutes, the contractor will be alerted by the EEY RN as soon as is reasonably possible.) The contractor shall be available for seasonal employee physicals generally late June or early July. 90% of the seasonal employees physicals shall be completed during the revetment deployment activation window as stated above. 3.GENERAL INFORMATION Ensley Engineer Yard is an industrial/marine facility operated by the U.S. Army Corps of Engineers, Memphis District, 2915 Riverport Road, Memphis, TN. It provides berthing for the District's marine fleet that includes 1 dredge, 3 tenders, floating plants, barges, and a variety of small craft. Our marine maintenance division operates 3 dry docks and a mile long marine terminal. Also housed are a variety of maintenance shops that include welders, painters, heavy equipment mechanics, small equipment mechanics, electricians, carpenters, pipe fitters, and machinists. In addition to the crafts mentioned above, other activities include the operation of cranes, trucks, and other heavy equipment, and the erection, use, and dismantling of scaffolding. Most scheduled physicals and river activities will cease by mid-November. Unless other needs dictate, medical services will be scheduled as needed. As a general rule, medical services are not required from mid-November, December and January. Due to the industrial nature of the work, both compliance-driven and general occupational medical surveillance examinations are conducted an average of 250 medical surveillance physicals per fiscal year. The USACE Safety Office will coordinate schedules and provide other assistance as needed. Contractor shall submit completed clause 52.212-3, Offeror Representation and Certifications, Commercial Items. The following clauses and provisions are incorporated and will remain in full force in any resultant award.
