- The purpose of this amendment for Solicitation No. AHRQ-11-10007-A is to clarify the following items: 1. Page 1 of the Solicitation is changed to read: "Your technical proposal must be concisely written and should be limited to 100 typewritten pages (double-spaced), inclusive of personnel qualifications (i.e., resume, etc., see Section L.9 for additional details). 2. Page 61 of the Solicitation is changed to read Part J - List of Attachments: Proposal Intent Response Form 1 pg 3. Page 78 of the Solicitation under c. Separation of Technical, Past Performance Information, and Business Proposal is revised to read: (1) Technical Proposal; (2) Past Performance Information; (3) Small Disadvantaged Business Participation Plan ; and (4) Business Proposal. 4. Page 98 under Section E is revised to read: • labor skill mix, • personnel selection and assignment, • system for monitoring and quality control of services provided; • plans for resolution of managerial problems; • ability and flexibility to respond to changes in budget, priorities, and schedule; delineation of lines of authority and delineation of staff responsibilities; • include organizational chart and staffing plan; • clear description of coordination of proposed subcontractors, including monitoring of performance; • description of communication methods that shall be used to ensure smooth communication, coordination and oversight; quality assurance; timely reporting; a high level of responsiveness to project directors' need and concerns. • And signed agreements between offeror and personnel (other than current direct employees) that includes dates of employment and specific tasks to be performed.
