- Patien Sensitivity Plant Inventory Performance Work Statement Request for Quotes This is a combined synopsis and solicitation for the following commercial items: MAINTAIN MEDICAL GROUP PLANTS FROM 01 OCT 11 THROUGH 30 SEP12 IAW attached SOWQty= 12 mo. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F1R4MD1116A001. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This procurement is being issued as 100% WOMEN OWNED SMALL BUSINESS SET-ASIDE. North American Industrial Classification Standard 561730, Size Standard, $7.0 Milllion, applies to this procurement. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 97-14 and Defense Acquisition Circular 91-13. FOB: Destination for performance to 7 MDG 697 LOUISIANA DR., DYESS AFB, TX 79607-1581. The provisions that apply to this solicitation are as follows: FAR 52.212-1, Instructions to Offerors--Commercial Items, FAR 52.212-2, Evaluation-- Commercial Items, and FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items. The offeror must include a completed copy of these provisions with their proposal. The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders--Commercial Items, applies to this solicitation.
