Attachment 5 - Disclosure of Ownership Attachment 4 - ORCA Attachment 3 - Price List Attachment 2 - FFM Evaluation Sheets Attachment 1 - PWS This is a combined synopsis/solicitation prepared in accordance with Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is not to be construed as a commitment by the Government. All information to be submitted at no cost to the Government. The solicitation will be issued as a Request for Quotation (RFQ) under the solicitation number F2CFEB1028A001. The solicitation includes the following attachments: 1. Performance Work Statement (PWS) 2. Repair of Folded Fiberglass Mat (FFM) Evaluation Sheets (40 ea.) 3. Price List for the Repair of Folded Fiberglass Mat's (FFM's) 4. Offeror Representation and Certifications 5. Disclosure of Ownership or Control by the Government of a Terrorist Country This solicitation document, and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-51, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 2010-0927, and Air Force Acquisition Circular (AFAC) 2011-0421. This is a commercial acquisition and it is a 100% Total Small Business Set-aside; all responsible small businesses may submit a quote. The applicable NAICS code is 325211, and the small business size standard is 750 employees. The government is soliciting a firm-fixed price (FFP) contract. Offers and award will be made on an all or none basis. The contractor shall provide all tools, materials, personnel, labor, transportation, and supervision and other required items necessary to repair FFM's in accordance with (IAW) the Engineering Technical Letter (ETL) 07-10, dated 19 December 2007. FFM repairs will consist of panel replacement, panel patching and hinge replacement. This RFQ contains TWO (2) Contract Line Item Numbers (CLIN's): CLIN 0001: Repair of Folded Fiberglass Mats. Repairs will be in accordance with ETL 07-10, dtd 19Dec07 (http://www.wbdg.org/ccb/AF/AFETL/etl_07_10.pdf). Each individual mat will require one and/or all of the following type of repairs: 1) Panel Replacement; 2) Panel Patching; and 3) Hinge Replacement. Qty: 40 each. CLIN 0002: Shipping. Transportation of FFM's from Tracy, CA to Contractor's repair location and back to Tracy, CA after all repairs have been completed. MARK FOR address shall be provided at time of award. Delivery shall be within 120 days after the order is placed with the successful offeror. All responsible sources may submit a quote, which will be considered by the Government. Offerors must ensure the provision FAR 52.212-3, Offeror Representation and Certifications-Commercial Items (Aug 2009), and its Alternate I (Apr 2002) (this may also be completed on http://orca.bpn.gov), DFARS 252.209-7001, Disclosure of Ownership or Control by the Government of a Terrorist Country (Jan 2009) and DFARS 252.212-7000, Offeror Representation and Certifications-Commercial Items (June 2005) are included with the quotation (Reference Attachment 4). Clauses and provisions may be accessed via the Internet, https://farsite.hill.af.mil or http://www.arnet.far.gov. All referenced Federal Acquisition Regulation (FAR) clauses and provisions in this solicitation may be accessed via the internet at https://farsite.hill.af.mil. The FAR provision 52.212-1, Instruction to Offerors-Commercial Items (Jun 2008), applies to this acquisition. Addenda to the following paragraphs of 52.212-1 are: (b) All quotes are due on or before 1200 hours, Hawaii Standard Time (HST), THURSDAY, 30 June 11. Submission may be made via email to curt.mutobe@hickam.af.mil, or via facsimile at 808-448-2911. Quotes must reference the solicitation number. Requirements: (1) Technical Capability. (a) Certified and approved by either of the officials/organizations referenced in paragraph 7.1.1 of the ETL 07-10, dtd 19 Dec 07 to make the following FFM repairs: • Panel Replacement • Panel Patching • Hinge Replacement (2) Price List for the Repair of Folded Fiberglass Mat's (FFM's). The contractor shall submit pricing for contract line item (CLIN) 0001 identified in the solicitation on the Price List at Attachment 3. This Price List will be incorporated into the resultant contract. (3) Warranty. Warranty of FFM repairs. BASIS FOR AWARD. Award will be made on an All or None basis to the successful offeror based on the following: (1) Quote meets Submission Requirements as stated above (2) Offeror who is authorized and certified to repair FFM's in accordance with ETL 07-10, dtd 19Dec07. (3) Offeror provides the lowest price to the Government (k) Central Contractor Registration (CCR). Offerors must be registered in the CCR database to receive a contract award.
