- Brachytherapy Prostate Implant Program Statement of Work 1.Service to be procured Brachytherapy Implantation of I-125 radioactive seeds for patients with prostate cancer. 2.All work will be performed on-site at the VA Medical Center, Cincinnati, OH. 3.Will require a team to include Radiation Oncologist (Authorized User [AU] and Medical Physicists) capable of being placed on the VA Nuclear Medicine permit. 4.The Brachytherapy team will provide ongoing guidance and assistance in maintaining and improving the brachytherapy program at the Cincinnati VA Medical Center. 5.Vendor must follow the ACR Guidelines and NHPP Guidelines (see attachments). 6. Procedure charges will be based on an hourly rate for physicians (Brachytherapists) and physicists not to exceed the Center for Medicare Services (CMS) rates. The Cincinnati VAMC estimates are 10-80 procedures per year. Physician Component (Brachytherapist): 1. The brachytherapist will review all prospective patients for possible prostate implants and confer with referring physicians as needed. This requires use of the VA electronic patient record as well as the use of Vista Web. 2.The brachytherapist will complete an Ultrasound for treatment planning for each implant patient. 3.The brachytherapist will prepare a treatment plan and provide a signed copy to the VA Nuclear Medicine Service to perform seed loading. 4.The brachytherapist will complete all components of the implant procedure in the operating room or other approved designated procedure room on site at the VA Medical Center. 5.The brachytherapist must be able to meet all requirements mandated by the VHA National Health Physics Program (NHPP), as well as Nuclear Regulatory Commission (NRC) requirements. 6.The brachytherapist may perform other types of brachytherapy when clinically indicated, such as eye plaques (Ophthalmology) and lung implants (Thoracic Surgery). The Cincinnati VA Medical Center currently has the equipment and clinical expertise required in these specialties outside of this prostate contract. 7.Because the brachytherapy program is subject to external reviews by the NRC, VA NHPP and possibly the Office of Inspector General (OIG), an hourly rate will be paid for these services. Estimated quantity will be ten hours per year, if an inspection occurs. A sign-in and sign-out sheet will be kept by the COTR. 8.Residents/Fellows will be supervised by the Brachytherapist during all implant procedures. Physicist Component: 1.The physicist will complete acceptance testing of the Treatment Planning System (10 CFR 35.457) to meet the American College of Radiology (ACR) requirements and provide a signed copy to the Radiation Safety Officer (RSO). 2.The physicist will certify each year, or as required after system maintenance or system upgrades, the Ultrasound unit and Image transfer from CT to the Treatment Planning System (TPSS). A signed copy of the certification(s) is to be provided to the RSO. 3.The physicist will annually review calibration measurements, or as required after system maintenance or system upgrades, for sealed sources to meet 10 CFR 35.432 requirements. Will provide a signed written copy to the RSO. 4.The physicist must be available on-site for unannounced inspections by the Nuclear Regulatory Commission (NRC), and the NHPP on a short notice. 5.The physicist will complete acceptance testing of the Treatment Planning System (10 CFR 35.457) to meet the American College of Radiology (ACR) requirements. A copy will be provided to the RSO. 6.The Physicist will complete quality assurance requirements for all other brachytherapy procedures, such as eye plaques and lung implants to meet the American College of Radiology (ACR) standards. A copy will be provided to the RSO. 7.Because the brachytherapy program is subject to external reviews by the NRC, VA NHPP and possibly the Office of Inspector General (OIG), an hourly rate will be paid for these services. In this instance, the physicist will be required to document his time and the number of hours devoted to the material review. Estimated quantity will be ten hours per year if an inspection occurs. Also, a sign-in and sign-out sheet will be kept by the COTR.
