- The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51. The solicitation number is W91YTV-11-T-0203, and the solicitation is issued as a Request for Quotation. This solicitation will result in one or more awards of a Blanket Purchase Agreement to vendor(s) determined to be responsive and responsible. The NAICS code is 811219 and the size standard is $19 Million. This solicitation is a 100% Small Business Set-aside. Blanchfield Army Community Hospital, Fort Campbell, KY has a requirement for replacement parts for various Government owned medical equipment. Attached to this synopsis is a listing of all known replacement parts (Attachment 2) for the current medical equipment. However, this list is not all inclusive and can be modified as new equipment is acquired. All replacement parts shall meet OEM specifications and shall be new or refurbished. All replacement parts shall have a minimum 90-day warranty period. Prospective contractors shall submit evidence of their ability to provide the replacement parts. All shipping shall be included. Prospective contractors shall submit any discounts available to the Government in their proposal. To evaluate pricing, Attachment 1 contains a representative list of items for which the prospective contractor shall quote. This priced list will be used to evaluate contractors pricing abilities based on historical data. The pricing in the quote will be used for future purchases of those replacement parts. Additionally, future purchases of parts shall establish the price paid by the Government for subsequent future purchases. The contractor shall provide the Government with 30-days notice of any price changes. Period of performance is 1 July 2011 through 30 June 2016. It is the intent to award multiple blanket purchase agreements. All requests for parts which exceed a combined value of $3,000.00 will be placed on a competitive basis by obtaining quotes from all BPA holders. The maximum call limit is $25,000.00 with a total maximum authorized limit of $150,000.00. Payment shall be made utilizing the Government Purchase Card.
