Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation, FA2517-11-T-6036, is being issued as a Request for Quotation (RFQ). (iii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-51. (iv) This procurement is procured as 100% Small Business Set Aside. The NAICS code is 337127, Institutional Furniture Manufacturing, and the size standard is 500 Employees. A single award will be made as a result of evaluation of quotes. (v) This requirement is for the purchase of Medical Shelving and installation at the 21st Medical Clinic, Peterson AFB, in accordance with the attached technical specifications and the Performance Work Statement (PWS). (vi) The medical shelving according to the PWS. (vii) The medical shelving shall be installed at the 21st Medical Clinic located on Peterson AFB CO. A site visit for this requirement will take place on 10 May 2011, 10:00 am MST. Due to increased security requirements, notification of attendees to SSgt Jeremy Rash and Ms. Kristin Davis is required NLT 6 May 2011, 1:00 pm MST. The required information must be submitted via email. Information needed on each individual to attend the site visit is as follows: Full Name, company represented, date of birth, place of birth, and last four of social security number. (viii) The following provisions and clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors-Commercial Items (Jun 2008). All firms must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. All firms must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. The Government is not providing contract financing for this requirement. Contractor shall submit design drawings to scale and detailed in 3-D format. With each design the contractor shall compile a Bill of Materials and complete floor plan drawing of the design. Supporting documentation shall be submitted to ensure technical specifications in Attachment 1 of the performance work statement are describle in detail. (ix) FAR 52.212-2, Evaluation-Commercial Items (Jan 1999), applies to this acquisition. Paragraph (a), award will be based on low price meeting technical specifications. To be considered technially acceptable, the design, floor plan, bill of materials and supporting documentation must comply with the technical specifications. (x) The provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items (Mar 2011), is applicable to this acquisition. All vendors shall be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov) at the time of Quote submittal. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (June 2010), applies to the acquisition. (xii) The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items (Deviation) (Mar 2011) applies to this acquisition.
