***** Begin Inserted Text Document - 'BLANKFBOSYNOPSIS.TXT' ***** THE MINNEAPOLIS VA HEALTHCARE SYSTEM ANTICIPATES A REQUIREMENT FOR GROUND AMBULANCE SERVICE BOTH BASIC LIFE SUPPORT AND ADVANCED LIFE SUPPORT TRANSPORTATION FOR ELIGIBLE BENEFICIARIES AT THE TWIN PORTS OUTPATIENT CLINIC IN SUPERIOR WISCONSIN. V-TRANSPORTATION NORTH AMERICAN INDUSTRY CLASSIFICATION SYSTEM (NAICS) IS 621910 SIZE STANDARD 7.0 MILLION Solicitation VA-263-11-RP-0164. The contractor shall furnish all labor, vehicles and materials, be fully licensed, insured, equipped and staffed to provide Ground Ambulance Services to transport scheduled (non-emergent) beneficiaries as directed. The contractor shall respond in accordance with the proposal instructions outlined in the solicitation. Performance period will be from time of contract award through end of the 12 month base year period, followed by two (1) year option periods. The solicitation VA-263-11-RP-0164 will be issued Unrestricted under NAICS code 621910. All qualified offerors are invited to provide their proposal. Offers, including options shall be evaluated for fair and reasonable pricing, technical competency and satisfactory past performance. The Government anticipates award of a commercial item contract to the offeror whose offer provides the best value to the Government. Award will be made utilizing a combination of FAR part 12,13,15 and 17 award procedures. The solicitation is scheduled to be issued on or about May 20, 2011 and will be issued via Fed Biz Opp.Electronic posting System at http://www.fedbizops.gov Offerors are responsible for checking the web site regularly to obtain amendments. Questions regarding this solicitation must be in writing. They may be sent by email to allen.brown1@va.gov or by fax to 612-333-3792, no telephonic inquiries will be accepted. Based on the current release date proposals must be in writing to the VA Medical Center Contracting office, located at 708 South 3rd St. Suite 200E, Minneapolis,Mn 55415-1116 on June 17,2011 at 3:30PM CDT, proposals must be in accordance with the proposal preparation instructions outlined in the solicitation. No web based offers will be accepted. Prospective contractors must be registered in the Central Contractors registration (CCR) database in order to obtain access in FedBiz Ops. CCR and if applicable VetBiz registration/certification must be current and active for contract award.
