- The sole source determination is based on the fact that the mission of the Sickle Cell Vascular Disease Section of the NHLBI Cardiovascular and Pulmonary Branch is to develop new knowledge and treatments for sickle cell disease. A current focus of our program is pulmonary hypertension in sickle cell disease. This is a form of high blood pressure in the lungs that is associated with a ten-fold increase in risk of mortality in adults with sickle cell disease. We are gaining clinical experience with several different drugs for pulmonary hypertension and submitting a new clinical research protocol for two drugs that have not previously been used in sickle cell pulmonary hypertension. As part of this effort the Government is looking for consultative medical experience of the type that is detailed in the specifications. The consultant should be available to attend clinic weekly at the NIH Clinical Center, without additional reimbursement for travel. The consultant should also be available by telephone around the clock for 44 weeks per year. Adults with sickle cell disease and pulmonary hypertension have extremely complicated medical problems, and expert experience is required for their safe and competent medical management. It is a highly specialized field and there are a few physicians in the US with strong experience in this area. The Government requires a consultant with sophisticated expertise and experience in tertiary medical care of complex adult sickle cell disease patients with life-threatening pulmonary hypertension and related medical complications, such as chronic renal failure, priapism and cutaneous leg ulceration. These specifications are fulfilled by Dr. Oswaldo Castro. Therefore it is in the Government's best interest to award an order to Dr. Castro to provide the above as outlined in the specifications above.
