- Evaluation Criteria for Support and Impact Monitoring of ESRD THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION: A SPEARATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQESTED. The Centers for Medicare and Medicaid Services (CMS) intends to award using Simplified Acquisition Procedures, a fixed price purchase order at a fixed price to purchase Support and Impact Monitoring of the End Stage Renal Disease Program during the ongoing implementation to a new set of regulations. "All business proposals final price shall be submitted in whole dollars only, rounded to the nearest dollar." The North American Industry Classification Code (NAICS) is 621399. The period of performance is one base year and two option years. The Request for Quote (RFQ) is being set aside for small business concerns. Responses must be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov Responses shall be no more than 10 pages and shall include: 1) Contractors capabilities overview 2) Past performance information 3) and Price of the deliverables, base year and Option years. The Government intends to award an order to the responsible offeror whose quotation offers the best combination of technical capabilities and proposed price; the best value (considering price, and other factors). Technical evaluation criteria has been provided in the attachment. All responses/quotes submitted shall remain valid for a period of 180 days. Interested parties must be able to affirm no conflicts of interest with parties involved in Medicare claims processing in light of the work identified in this Statement of Work. In the event that the vendor/government identifies a conflict, the vendor shall be responsible for providing a mitigation strategy that satisfies the government. In the event that the conflict cannot be resolved, the order cannot be awarded or an existing order may need to be terminated. Questions and inquiries must be submitted in writing no later than 3:00pm on May 17, 2011 to: phillip.harrell@cms.hhs.gov. Responses and clarifications will be provided in an amendment to the solicitation by May 19, 2011.
