Description
REQUEST FOR PROPOSAL Dust collection South Bronx Job Corps Academy is soliciting bids for the Dust collection system at their facilities, located at 1771 Andrews Ave, Bronx, New York. The South Bronx Job Corps Academy is a federally funded employment training program that services approximately 200 residential and 100 non-residential trainees. The program is open year round and is operated by ResCare, Inc. for the United States Department of Labor. SCOPE OF WORK: Th Room: BAM shop dust collection system is obsolete and no longer functional. Replace the existing dust collection system with two (2) portable dust collectors having two-stage dust collectors. The dust collectors shall have one horsepower, single phase motor/blower unit with dust/filter bag, hose, intake nozzles, and 55 gallon drum on a wheeled base. REQUIREMENTS •· Bidder must call for appointment to view the project, only bidders that viewed the project will be consider Must be in compliance with all Local, State and Federal Guidelines and Regulations •· Work to be accomplished in safe and efficient manner with minimal or no disruption or endangerment to trainees and / or staff. •· must provide a copy of current licensure and insurance LENGTH OF AGREEMENT Contractor must provide scope of work and time frame in which work is to be completed. ACKNOWLEDGEMENT OF ACCEPTANCE •· Contractor must acknowledge by signature that they meet all Federal, State, Local and OSHA regulations and that all specifications terms and conditions set forth are acceptable. •· Bidders must submit their proposal in a sealed envelope by 5 p.m. close of business on May 29, 2011 to the attention Vivian Lopez, Purchasing Agent. The following documents Davis Bacon Act (40 U 27a Attachment B), Certification of Non-Segregated Facilities Pursuant to FAR 52.222-21 (Attachment C), Equal Opportunity and Affirmative Action Requirement Pursuant to FAR 52.222-26 and FAR 52.222-27, FAR 52.252-2, 52.204-1, 52.215-19, PRC 4121 and 4170 will be incorporated and made part of agreement. All questions regarding this matter can be directed to Vivian Lopez 718-294-7530 or lopez.vivian@jobcorps.org, Purchasing SAFETY AND HEALTH: All work shall be accomplished in a safe and efficient manner, with minimal disruption to the operation of the Academy. The subcontractor represents, by acceptance of any resultant agreement that they are in compliance with all regulations and standards flowing from the Occupational Safety and Health Act of 1970 (P.O. 91-596, 29 USC 651 et seq.) and further that in the event hazardous materials are involved, will comply with Federal Standards No. 310 dated July 1971 to the latest change, entitled "Symbols for Packages and Containers for Hazardous Chemicals and Materials." Vendor acknowledges that by acceptance of any resultant agreement, that all work and materials used in the performance of the contract duties meet federal, state, local and OSHA regulations. PROPOSAL SUBMISSION: Interested parties should respond to the above solicitation by the COB on May 29, 2011 to be eligible for award. Please submit your proposal and all signed attachments via express mail or other traceable mailing method and reference the notice number on your envelope.
