Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This RFQ will also use the procedures under FAR Part 13. In accordance with 13.106-2, the contracting officer may take into consideration information such as the contracting officer's knowledge of and previous experience with the supply being acquired or other reasonable basis. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference/solicitation number is VA-263-11-RQ-0259 and the solicitation is issued as a request for quote (RFQ). The combined synopsis/solicitation will result in a contract and corresponding purchase order. The provisions and clauses incorporated into this solicitation document are those in effect at the time of publication. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. This requirement is for 2 Smith & Nephew Arthroscopic Kits US 560P. The equipment will be used to provide surgical care to our patients and is needed for compatibility with equipment already purchased. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-38. The factors that will be evaluated include, in no specific order of precedence: a) Meeting Technical specifications, b) Past Performance, c) Experience & Qualifications, d) Cost, e) Warranty and Service Delivery Location: VAMC St. Cloud, Warehouse, Bldg 14, 4801 Veterans Dr. St. Cloud, MN 56303eye Clinic) Delivery and acceptance of deliverables will be FOB destination within 30 days of receipt of order to VA Medical Center, Bldg 14, 4801 Veterans Dr. St. Cloud, MN 56303 The provision at FAR 52.212-1 Instructions to Offerors - Commercial Items, applies to this solicitation and incorporate the following addenda to 52.212-1, 52.204-6 Data Universal Numbering System (DUNS) Number; VAAR 852.219-70, 852.233-70 Protest Content; 852.236-76 Correspondence; 852.237-70 Contractor Responsibilities; 852.270-1. The provision at FAR 52.212-2, Evaluation - Commercial Items applies to this requirement. The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Capability to meet the governments requirements, Past Performance, Price and price related factors. Equipment will be used by multiple physicians; and will thereby be evaluated for use in a multi-physician environment. Other considerations include equipment ability to minimize thermal damage, cutting efficiency, and ability to utilize standardized tips. The provision at 52.212-3, Offeror Representations and Certification, Commercial Items, applies to this solicitation. The contractor shall either: (1) return a completed copy of this provision with its quote (a copy of the provision may be attained from http://www.arnet.gov/far); or (2) complete the Online Reps and Certs.
