Solicitation Number: W91247-11-R-0016 Notice Type: Pre-solicitation Notice - Survivor Outreach Services (SOS) The Mission and Installation Contracting Command (MICC) Center - Fort Bragg, North Carolina anticipates issuing a Request for Proposal (RFP) for a Firm Fixed Price (FFP) contract for Contractor provided Survivor Outreach Services. Place of Performance: The Contractor will provide SOS program services Army-wide at designated Continental United States (CONUS) locations, to include Fort Bragg, NC; Houston, TX; Los Alamitos, CA; Moffett Field, CA; Charlotte, NC; Fort Jackson, SC; Orlando, FL; Miami, FL; Belle Chase, LA; Fort McCoy, WI; Darien, IL; Wichita, KS; Salt Lake City, UT; Seattle, WA; Rochester, NY; Coraopolis, PA; Fort Dix, NJ; and Atlanta, GA. General Information: The Government intends to award a FFP contract. This requirement is restricted to the 8(a) program on a competitive basis, in accordance with FAR Subpart 19.8. The acquisition is being offered for competition limited to eligible 8(a) concerns. All other offerors are ineligible to provide proposals. The SOS program will provide short term and long-term case management, (network of providers); benefits counseling; financial planning; legal assistance; personalized and integrated web-based support; one-stop service provider; service delivery based on specific individual needs; support as long as the Family member wants/needs support. The SOS provides substantial support to the Casualty Assistance Officer to ensure Families receive the most current information and have access to long-term financial and emotional support. The Contractor will establish support services, crisis response and intervention, grief care and counseling resources, casework assistance, long-term Survivor wellness, and community and military outreach. The Contractor will provide all personnel, management, tools, supplies, supervision, transportation, labor, equipment, and other items and services necessary to provide SOS program support services. SOS services are available to all Survivors, not just Survivors of the current conflict, and are available until the Survivor desires to discontinue services. In addition, the Army Reserve SOS staff will provide awareness education to the military community about the needs of Survivors and maintain linkage between Survivor and the Army. The North American Industry Classification System (NAICS) code for this procurement is 624190, for other Individual and Family Services, with a size standard of $7 Million. The Government anticipates the Period of Performance (POP) to include a fifteen (15) day phase-in from 15 July 2011 through 31 July 2011, a base period of 1 August 2011 through 30 June 2012, and one (1) 1-year option period. The anticipated posting date of the RFP is on or around 25 April 2011. Site Visit Information: No site visit will be conducted due to USAR SOS multiple locations.
