Description
Wage Determination Additional Terms and Conditions (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 11-236-SOL-00041 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-51. (iv) The North American Industry Classification System (NAICS) code is 561920 with a small business size standard of $7M. Under the authority of the Buy Indian Act, this acquisition is 100 percent set aside for competition among Indian-owned vendors. (v) The Contract Line Items (CLINS) are as shown below. See the attached, Enclosure 1, Schedule of Prices for the details. CLIN 0001 thru 0004 (Base Period) – The Contractor shall provide all labor, materials and equipment to perform the services for the event as indicated in the attached Statement of Work. CLIN 0005 thru 0006 (Option Period 1) – The Contractor shall provide all labor, materials and equipment to perform the services for the event as indicated in the attached Statement of Work. CLIN 0007 thru 0008 (Option Period 2) – The Contractor shall provide all labor, materials and equipment to perform the services for the event as indicated in the attached Statement of Work. (vi) The Indian Health Service (IHS) Division of Behavioral Health (DBH) annual National Behavioral Health Conference is a training, education, and skill building event serving Indian Country. The main objectives of the national event are to create opportunities for networking and collaboration, provide the most up-to-date information on best and promising practices, increase awareness and understanding of current and emerging behavioral health problems and pursue professional development opportunities. This training event provides a variety of modes for learning through subject matter plenary sessions, panels, keynote addresses, innovative hands-on learning labs, and workshops focused on culturally competent approaches to services. The IHS DBH requires an event management coordinator to assure appropriate support is provided for meeting preparation, logistics planning, material development and distribution, onsite event facilitation and post-event clean up. (vii) n/a (viii) FAR 52.212-1 - Instructions to Offerors - Commercial Items (June 2008): Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: 1. Solicitation number. 2. Name, address, telephone number of the offeror and email address of the contact person. 3. Technical description of the item/service being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. See the attached, Enclosure 4, Quotation Submission Requirements. 4. Price and any discount terms. 5. "Remit to" address, if different than mailing address. 6. A completed copy of the representations and certifications at FAR 52.212-3. 7. A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (ix) 52.212-2, Evaluation – Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The offeror must include in their quotation, the unit price, the list price, the performance period after contract award, the prompt payment discount terms, the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Note: In order to receive an award from the Indian Health Service, contractors must have a valid registration in the Central Contractor Registration (CCR) www.ccr.gov. (xiv) n/a (xv) Quotations must be receipted by the IHS no later than April 22, 2011, 4:00pm Eastern Standard Time. (xvi) Interested vendors capable of furnishing the government with the item specified in this combined synopsis/solicitation should submit their quotation to the below address or email address.
