- Description
- The U.S. Forest Service, Golconda Job Corps Center, located at Route 1, Box 104A, Golconda, IL 62938, is a residential, educational and vocational training facility for student's ages 16 - 25 years of age. This is a request for quotes (RFQ) and the Contractor shall provide Mental Health Services under this contract. This requirement will be solicited and awarded in accordance with the Acquisition of Commercial Items, Federal Acquisition Regulations (FAR) Part 12. It is Set-Aside for 100% Small Business. The NAICS code is 621112 and the small business size standard is $10 million. Contractors must be registered in the Central Contractor Registration Database located at http://www.ccr.gov, and must complete electronic representations and certifications at the ORCA database located at http://orca.bpn.gov. All responsible sources may submit a quote for consideration. The performance period is 365 days beginning July 1, 2011 through June 30, 2012, for the base period, with four (4), 12-month option periods which may be exercised at the Government's discretion. Personnel performing the services must be certified and/or licensed in the State of Illinois and demonstrate appropriate insurance coverage. The Government plans to award a firm fixed-price contract resulting from the solicitation to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government based on the evaluation factors listed in the solicitation. This pre-solicitation notice does not constitute a solicitation. The solicitation will be posted to this website for downloading and will provide all necessary information for quotes to be prepared and submitted for consideration. Quoters shall monitor this website for the solicitation. Paper copies of the solicitation will not be issued and telephone requests or fax requests for the solicitation will not be accepted. Quotes will be accepted at the address listed, to the attention of Nikoletta E. Klimak, Contract Specialist, and address: U.S. Forest Service, 740 Simms Street, Golden, CO 80401. Electronic quotes will be accepted. Faxed quotes are not acceptable. The Government reserves the right to cancel this solicitation. All questions related to the solicitation must be submitted in writing to Nikoletta E. Klimak at neklimak@fs.fed.us. Only written requests will be considered and addressed.
