- The Womack Army Medical Center Contracting Office is considering whether or not to set aside an acquisition for either HUBZone concerns, or for Service-Disabled Veteran-owned Small Business concerns (SDVOSB) for Certified Registered Nurse Anesthetists (CRNA). The NAICS code is 621399 (Offices of all Other Miscellaneous Health Practitioners) with the standard dollar amount of $7.0. The contractor shall provide CRNAs for Womack Army Medical Center. The requirement is for: Fourteen (14) full time equivalents (FTE) certified positions in the Operating Room (OR), and four point six (4.6) FTEs certified in Labor and Delivery The proposed requirement will be for base and four year options. The proposed performance date will be effective 1 September 2011. To submit an offer on this acquisition, your firm shall be compliant with the Federal Acquisition Regulations. If your firm is HUBZone certified your applicable regulation is 19.1303, if your firm is Service-Disable Veteran-owned Small Business, your regulation is 19.1405. Submittals: (a)A positive statement of your intention to submit a proposal for this solicitation as a prime contractor (b)Evidence of experience in work similar in type and scope to include contract numbers, project titles, dollar amounts, points of contact and telephone numbers (c)Past performance references with points of contact, telephone numbers, and email address. At least two (2) references are requested, but more desirable (d) CCR information Your response is required by 11 April 2011 at 1400 hours, Eastern Standard Time. Contractors are reminded that should this acquisition become a HUBZone set aside, FAR 52.219-3 applies, and if set aside for Total Hubzones, at least 50 percent of the cost of personnel for contract performance be spent for employees of the concern. Contractors are reminded that should this acquisition become a Service-Disabled Veteran owned Small Business Set-Aside, FAR 52.219-27 applies, Notice of Total Service-Disabled Veteran Owned Small Business Set-Aside, which requires at least 50% of the cost of personnel for contract performance be spent on employees of the concern. All of the above information must be submitted in sufficient detail for a decision to be made on a HUBZone or Service-Disabled-Owned Small Business (SDVOB) set aside. Failure to submit all information requested may result in unrestricted acquisition.
