Description

Synopsis: This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for proposal or quote and does not obligate the Government to award a contract. Request for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owed Small Businesses, 8(a), Hub-Zone and other Small Businesses interested and capable of performing the work. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, email (e) DUNS number (f) Cage code (g) TAX ID number (h) Type of small business, e.g. Service Disabled Veteran Owed Small Business, Veteran Owed Small Business, 8(a), HUB-Zone, Women Owned Small Business, Small Disadvantaged Business, or Small Business HUB-Zone business and (i) must provide a capability statement that addresses the organization's qualifications and ability to perform as a contractor for the work described below. Description: The New Jersey VA Healthcare System, 385 Tremont Avenue, East Orange, NJ 07018 is seeking firms that can provide Telehealth Equipment Support Services to the beneficiaries of NJ. This service will be established for all counties in New Jersey where service is available. NJ VA Health Care System requests the contractor to be tasked with the following: 1.Orders for monitors will be processed by the contractor's Customer Service Department. 2.Specially trained technicians will pick up monitors at the NJ VA and delivered to the Veteran's home within 24 hours of the order. 3.The technician will provide specific instructions to the Veteran and/or caregiver, on the operation of the equipment. 4.The technician will install and test equipment as per manufacturer's procedures. 5.Upon request, equipment will be picked up from the Veteran's home within 72 hours of call. Each piece of equipment will be handled using universal precautions. Equipment will be cleaned before returned to the NJ VA Health Care System. 6.Product troubleshooting and /or replacement will be provided at the home as requested. 7.Equipment not operating within manufacturers specifications will be returned to the NJ VA for repair or replacement. Telephone requests will not be honored.
