Attachment A - NCI-Frederick Building Guide Attachment 9 - NIH Notice and Authorization for Release of Information Attachment 8 - Hot Work Permit Attachment 7 - Payment Bond Attachment 6 - Performance Bond Attachment 5 - Chemical Usage Report Attachment 4 - Daily Progress Report Attachment 3 - Representations and Certifications Attachment 2 - Wage Determination Attachment 1 - Interior/Exterior Painting Contract Specification dated 2/11 Solicitation H11-1531 FIXED PRICE - REQUIREMENTS CONTRACT Contractor shall provide all labor, materials, stencils, equipment, supplies, and services to accomplish the painting indicated in the attached Specification "INTERIOR/EXTERIOR PAINTING CONTRACT, DATED 2/11". Interior/exterior painting services will include substrate repair, preparation and painting of interior and exterior surfaces as needed as NCI-Frederick. The base period of performance (CY 11) shall be from date of award through 9/25/11. There will be four (4) one year options, which may be exercised by SAIC-Frederick. Option 1 (CY 12) is from 9/26/11 through 9/25/12, Option 2 (CY 13) is from 9/26/12 through 9/25/13, Option 3 (CY 14) is from 9/26/13 through 9/25/14 and Option 4 (CY 15) is from 9/26/14 through 9/25/15. No pre-bid conference is scheduled. Offerors desiring a site visit may contact the Subcontracts Specialist. Written questions will be accepted until Friday, March 25, 2011. An amendment with responses to the questions will be posted on FBO. Performance shall be at the direction of the SAIC-Frederick Contracting Officer's Technical Representative (COTR). Orders exceeding $25,000 must be issued in writing and performance and payment bonds will be required.
