- Statement of Work and Justification Sole Source MFR This Request for Quote (RFQ) is for a commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number for this procurement is F2E3CS1048AC01. The anticipated award will be a Firm Fixed Price Contract. The award will be made based on price. This acquisition will be a sole source requirement IAW FAR 6.302. The North American Industry Classification System (NAICS) code is 517919. The SB size standard for this code is $25.0 million. Quotes will be due by 15 Mar 11 at 10:00 AM MST. DESCRIPTION: CLIN 0001 - Re-write computer programming script so the Telephone Management System (TMS) can recognize both 5 and 6 digit PINs for Hearts Apart and authcodes. Suggested Sorce: Ventraq Corporation. Provision at FAR 52.212-1, Instruction to Offerors - Commercial Items; FAR 52.247-34, FOB Destination; & FAR 52.211-6, Brand Name or Equal applies to this acquisition. The following addenda is provided from this provision: Offer should be submitted and shall contain the following information: SOLICITATION NUMBER; NAME, ADDRESS, TELEPHONE NUMBER OF OFFEROR; TAX IDENTIFICATION NUMBER; PRICE AND ANY DISCOUNT TERMS; DELIVERY TIMEFRAME; GSA CONTRACT NUMBER IF APPLICAPBLE; AND ACKNOWLEDGEMENT OF ALL SOLICITATION AMENDMENTS.
