Project 618-11-109 Fire Damper Testing RFP: VA263-11-RP-0129 Base Items: 1.Contractor to provide service to inspect and test approximately 500 fire and smoke dampers in Building 70, VA Minneapolis Healthcare System, One Veteran's Drive. Inspection shall meet NFPA code requirements. 2.Vendor is responsible for providing and installing all required ICRA barriers and complying with all required ICRA actions. 3.All tests shall be completed in a safe manner by personnel wearing personal protective equipment. 4.Assign each damper a unique identifying number. Place a sticker with this number on or near each damper during the inspection. 5.Tasks to be performed included: a.Locate and operate (i.e. actuate) all fire dampers and smoke dampers in the facility b.Remove and reset the fusible link, to verify the damper fully closes. c.Check for fully unobstructed access d.Ensure no interference due to rusted, bent, misaligned, or damaged frame or blades, or defective hinges or other moving parts e.Check for penetrations by foreign objects f.Check for anything blocking the closure in any way g.Lubricate all moving parts of the fire damper. h.If damper is provided with a latch, check operation of the latch. i.Replace all fusible links that are compromised (per NFPA Code). j.Ensure all dampers have identification stickers with identifying numbers. 6.Supply a Master Report Listing the following for each damper a.Location of fire or smoke damper b.Unique damper identifying number. c.Was the damper tested successfully (Y/N) d.If the fire or smoke damper failed inspection, provide a detailed reason. e.Corrective actions taken f.Date of testing g.Name of inspector 7.Provide Digital Documentation (i.e. Digital Picture) of every damper "Before" and "After" Inspection. 8.Provide a list of all inaccessible damper locations and recommended actions for access. Recommendations will include descriptions of work and cost estimate. 9.Contractor shall schedule maintenance and inspections; and coordinate access to all HVAC mechanical spaces and equipment, and shut down of equipment or systems with the COTR, Catherine Joyce, 612-467-5532. 10.Some dampers may be very difficult to access and require shutdowns and additional life safety and infection control measures. Contractor is responsible for arranging all access, providing all required construction barriers, and coordinating shutdowns so as to not interfere with continuity of operations. Contractor shall follow all OSHA and VA safety requirements when accessing dampers. 11.Contractor will give at least 10-day notice of all proposed shutdowns of air handlers and other equipment. Work will be done on premium time where necessary to minimize impact to critical care and patient operations. 12.Contractor shall follow all VA confined spaces policies and procedures. Contractor shall obtain all necessary permits and receive approval from the VA Safety Department before performing any testing in confined spaces. All contract employees who will be entering confined spaces shall receive confined space training deemed acceptable by VA safety personnel. Option Item 1: Please provide the cost to install the following types of access doors where the duct is accessible but has no access panel: Cam-Lock Door (8"x8")$/each__________ Cam -Lock Door (12"x12")$/each__________ Cam-Lock Door (14"x14")$/each__________
