Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ), solicitation number W912D0-11-T-10480471 in accordance with FAR Parts 12 and 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-49, effective 24 January 2011. The North American Industrial Classification System (NAICS) number is 541330 and the business size standard is $4.5 million. The proposed acquisition is 100% set aside for small business. Contractor Requirements: Fort Wainwright, Alaska intends to award a firm fixed price contract for the inspection/certification of 4 (ea) ski lifts. Three of the ski lifts are located on Fort Wainwright, Alaska, and the other is located at the Black Rapids Training area which is approximately 100 miles south of Fairbanks, Alaska. The contract will be for a base year plus two option years. LINE ITEM/DESCRIPTION: BASE YEAR CLIN 0001 Labor and equipment necessary to inspect and certify 4 (ea) ski lifts IAW the attached Statement of Work. Work is to be completed NLT 15 September 2011. Total Price__________________ CLIN 0002 Travel, lodging, and associated costs. Total Price__________________ LINE ITEM/DESCRIPTION: OPTION YEAR ONE CLIN 0001A Labor and equipment necessary to inspect and certify 4 (ea) ski lifts IAW the attached Statement of Work. Work is to be completed NLT 15 September 2012. Total Price__________________ CLIN 0002A Travel, lodging, and associated costs. Total Price__________________ LINE ITEM/DESCRIPTION: OPTION YEAR TWO CLIN 0001B Labor and equipment necessary to inspect and certify 4 (ea) ski lifts IAW the attached Statement of Work. Work is to be completed NLT 15 September 2013. Total Price__________________ CLIN 0002B Travel, lodging, and associated costs. Total Price__________________ Offerors must comply with all instructions contained in FAR 52.212-1, Instructions to Offerors--Commercial Items. FAR 52.212-1 is supplemented per following addenda: Contractor shall submit their quote on company letterhead, provide solicitation number, the time specified in the solicitation for receipt of offers, name, address, and telephone number of the offeror, cost proposals as indicated in the CLINS above, a list of recent jobs completed by the company that shows competency in completing the work as indicated in this combined synopsis, any discount terms, cage code, DUNS number, size of business, and Tax ID number. The provisions at FAR 52.212-2, Evaluation--Commercial Items apply to this acquisition. The Government will award a purchase order resulting from this solicitation to the responsive responsible offeror whose technically acceptable offer conforming to the combined synopses will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offers: Award will be made on the basis of the lowest evaluated price that conforms to all the requirements set forth in this combined synopses. The Government intends to evaluate offers and make award without discussions; however, this does not preclude the Government from having discussions if it is in the best interest of the Government to do so. The provision s of FAR 52.217-9, Option to Extend the Term of the Contract, applies to this acquisition. a) The Government may extend the term of this contract by written notice to the Contractor within 30 days of the start of the next option period provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 36 months.
