Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulations (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. (ii) Request for Quotation (RFQ) W9114F-11-T-0008 is hereby issued for non-personal, commercial services for the Armed Services Blood Bank Center-Europe (ASBBC-EUR), Landstuhl Regional Medical Center (LRMC), Germany. (iii) The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-48, effective 31 January 2011. The complete text of any of the clauses and provisions may be accessed in full text at https://www.acquisition.gov/far/ (iv) The acquisition will be conducted on a full and open competition basis. The associated NAICS code is 621511 "Medical Laboratories". The applicable small business size standard is $13.5 M. (v) The contract line items for this acquisition are listed in section B-1 "Schedule of Items and Prices" outlined in the attachment entitled "Additional Documentation", which can be found at the bottom of this announcement. (vi) The Government intends to issue a blanket purchase agreement (BPA) to provide Blood Donor Testing Services for the Armed Services Blood Bank Center-Europe (ASBBC-EUR) as described in Section B-5 "Performance Work Statement" outlined in the attachment entitled "Additional Documentation", which can be found at the bottom of this announcement. (vii) ASBBC-EUR will order services under the BPA by issuing individual BPA call orders. The total potential effective period of this BPA is 60 months from the start of the agreement unless otherwise terminated or extended. All prices are F.O.B. Destination. (viii) The provision at FAR 52.212-1, Instructions to Offerors-Commercial (JUN 2008), applies to this acquisition. The addendum to the FAR provision 52.212-1 is outlined in the attachment entitled "Additional Documentation", which can be found at the bottom of this announcement. (ix) The provision at FAR 52.212-2, Evaluation-Commercial Items (JAN 2009), applies to this acquisition. The addendum to the FAR provision 52.212-2 is as follows: 1.The Government intends to award a firm-fixed price BPA as a result of this RFQ on an all or none basis. 2.Evaluation and award will be made using the lowest price technically acceptable selection process on the basis of the lowest price, technically acceptable quote that is rated acceptable for all of the experience and performance capabilities. Technical acceptability will be determined from information submitted by the quoter, which must provide sufficient details to show that the proposed items meet the stated salient characteristics. Past performance will be evaluated as part of the responsibility evaluation of the apparent technically acceptable low quoter. 3.The Government will evaluate quotes as described at section E-3 outlined in the attachment entitled "Additional Documentation" that can be found at the bottom of this announcement. 4.The following evaluation standards will be used to rate quotes: a.Acceptable: Meets performance or capability requirements. A quote that satisfies all of the stated requirements and is complete. A quote that contains no major error(s), omission(s) or deficiency (ies). b.Unacceptable: Fails to meet performance or capability requirements. A quote that is incomplete. A quote that contains a major error(s), omission(s) or deficiency(ies) that indicates a lack of understanding of the requirements. (x) The provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (JAN 2011) ALTERNATE I (APR 2002), applies to this RFQ. The offeror shall complete the annual representations and certifications electronically at http://orca.bpn.gov. The provisions at DFARS 252.212-7000, Offeror Representations and Certifications- Commercial Items. (JUN 2005) applies to this RFQ. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (MAR 2009), applies to this acquisition. The additional clauses that shall also apply to any and all BPA call orders issued against the resulting BPA are listed in the attachment entitled "Additional Documentation", which can be found at the bottom of this announcement. (xii) The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (Aug 2009) (DEV) applies to this acquisition. The clause at DFARS 252.212-7001, Contract Terms And Conditions Required To Implement Statutes Or Executive Orders Applicable To Defense Acquisitions Of Commercial Items (JUL 2009) (Deviation), applies to this acquisition. (xiii) The items included in the quote shall be consistent with customary commercial practices. (xiv) The Defense Priorities and Allocations System (DPAS) rating is not applicable. (xv) Quotes shall be submitted no later than 8:00 p.m. Central European Time, March 14, 2011, by electronic mail to lourdes.schroeder@us.army.mil Maximum electronic mail size: 3MB per email.
