- DEPARTMENT OF VETERANS AFFAIRS (VA) SEEKS EXPRESSIONS OF INTEREST FOR APPROXIMATELY 15,000 NET USABLE SQUARE FEET FOR A LEASED COMMUNITY- BASED OUTPATIENT CLINIC IN EUREKA, CALIFORNIA REQUIREMENT: The VA is seeking to lease approximately 15,000 net usable square feet (approximately 20,250 gross square feet) plus parking to meet a ratio of 8:1,000 net usable square feet, or as required by local code, whichever is greater, for dedicated use by the Department of Veterans Affairs, San Francisco VA Medical Center for an outpatient clinic. The lease term shall be for a twenty (20) year firm term. Occupancy is required by the fall of 2012. LOCATION: The offered leased location must be in Eureka, California, and within the following boundaries: North: Humboldt Bay West: Humboldt Bay South: Herrick Avenue / Fairway Drive / F Street to Oak Street, east on Oak Street to H Street, north on H Street to Manzanita extended east through Redwood Acres Fairway to the extension of Hall Avenue East: Indianola Cutoff Road south to Myrtle Avenue continuing west to Hall Avenue Properties immediately adjacent to the boundaries will be considered to be within the delineated area. The VA reserves the right to amend the delineated area and/or lease requirements. EXPRESSIONS OF INTEREST: Brokers and/or agents may represent the owner, but leased locations submitted by brokers/agents must be accompanied by written evidence that they are authorized to represent the ownership of the property. EXPRESSIONS OF INTEREST SHOULD INCLUDE THE FOLLOWING INFORMATION, AS KNOWN OR AVAILABLE. PLEASE NOTE THAT TIME IS OF THE ESSENCE SO PLEASE PROVIDE AS MUCH INFORMATION AS POSSIBLE TO ENABLE VA TO EVALUATE YOUR OFFERED PROPERTY. VA RESERVES THE RIGHT TO ELIMINATE A PROPERTY THAT FAILS TO PROVIDE SUFFICIENT INFORMATION. 1. Property location and address 2. Rentable and net usable square footage offered 4. Asking Rental Rate 5. Date by which tenant improvements or alternations to the facility could commence 6. Existing Floor and Site Plan(s) 7. Specific information on ownership and evidence that offeror currently owns and controls the property 8. Provide evidence that the property offered is outside the 100 year flood zone and the Tsunami Inundation Zone. Clinic facility and grounds and landscaping must comply with VA Life Safety, Seismic, Radon and ADAAG requirements. Also, any persons or firms submitting or considering the submission of a property for lease are encouraged to contact the following with any questions or clarifications regarding this notice including the ability to respond to the information requested in this notice. A market survey of offered properties will be conducted by VA.
