Description
Brief memo describing functions to be performed by instructor. Pricing proposal to be submitted by perspective contractor(s). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA4800-11-Q-PD02. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-48, dtd 31 Jan 2011. This acquisition is designated for 100% Small Businesses. The associated NAICS code is 611620. Size standards in dollars is $7.0 million. This requirement consists of two (2) line items. Requirement is to provide one instructor to teach Yoga and Pilate classes at Fort Eustis, Virginia. See attached price list on RFQ FA4800-11-Q-PD02, dtd 16 Feb 2011. See attached Memo for Record, dtd 16 Feb 2011. PROVISIONS/CLAUSES. The provision at FAR 52.204-10, Reporting Executive Compensation and First-Tier Subcontract Awards applies to this acquisition. FAR 52.212-1, Instructions to Offerors-Commercial Item, applies to this acquisition. FAR 52.212-3 applies to this acquisition, requiring all offers are to be registered in CCR and ORCA. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this solicitation. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items, applies to this RFQ. The clause at DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders applicable to Defense Acquisition of Commercial Items, applies to this RFQ. Specifically, the following DFARS clauses cited are applicable to this solicitation: 252.204-7004 Alternate A, Central Contractor Registration, 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports, 252.243-7001 Pricing of Contract Modifications, 252.246-7000 Material Inspection and Receiving Report. The following AFFARS clause cited is applicable to this RFQ: 5352.201-9101 Ombudsmen and 5352.242-9000, Air Force Installations is applicable to this RFQ.
