- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Quota tions are do to Julio Cabrera (julio.cabrera@us.army.mil) via e-mail not later that 2 Feb 11 at 1500 hours. STATEMENT OF WORK (SOW) BEYOND THE HORIZON (BTH) 11 EL SALVADOR (SLV) RENTAL OF AUTOMATION PROCESSING DATA EQUIPMENT C.1. GENERAL. This is a lease/rental non-personnel service contract with maintenance and technical support, which is responsibility of the contractor C.1.1. OBJECTIVE. Contractor shall provide all personnel, transportation, and equipment necessary to provide Automation Data Processing Equipment (ADPE) as stated in paragraph C.2. at the Pol gono de Tiro in San Vicente, SLV. C.1.2. SCOPE OF WORKS. Contractor shall deliver, set-up and demonstrate the performance, serviceability, and functionality of the equipment specified below. Services shall be provided during the days specified herein. C.2. DESCRIPTION. C.2.1. CONTRACTOR FURNISHED EQUIPMENT: a. One (1) Color Printer with new black and color cartridges and two (2) spare set cartridges. PERIOD. 26 February thru 19 June 2011. C.4. PERFORMANCE REQUIREMENTS. Contractor shall be responsible for the cabling and installation/configuration of the equipment at Pol gono de Tiro in San Vicente, SLV. C.4.1. Delivery/Pick-up. Contractor shall deliver all equipment mentioned above no later than 0830 hours on the first day of the contracted period. Contractor shall pick up the equipment no earlier than 1800 hours the last day of the contract. C.4.2. Contractor shall inventory the equipment upon initiation of the contract with the Contracting Officer's Representative (COR) and thereafter, as necessary. COR will verify all equipment is accounted for IAW this SOW and that all equipment complies with the minimum requirements. C.4.3. Contractor shall provide a point of contact and phone number for on site technical support if needed. Support shall be an on-call basis from 0700 hours to 1930 hours daily during the contract period. C.4.4. Contractor shall respond to equipment malfunction within one (1) hour of notification. Repairs or replacement of malfunctioning equipment shall be accomplished within four (4) hours. The Contractor shall provide a 24/7 contact number for reporting malfunctioning equipment. C.4.5. Authority to change terms, conditions, standards, or contract cost, remains with the Contracting Officer (KO). C.4.6. Contractor shall be responsible for all equipment maintenance and supplies for operation and maintenance of the service required under the terms of this contract. C.5. LOSS OR DAMAGE BEYOND FAIR WEAR AND TEAR: Whenever equipment provided under the terms of this contract is returned to the Contractor with damages considered over and beyond fair wear and tear and it is the Contractor's intent to invoice the US Government for such damage or loss, the Contractor shall prepare and submit in writing a detailed estimate to the KO for the cost and labor and parts necessary to repair the equipment to its original condition when the US Government accepted it. Repairs to the damaged equipment will not be performed until the KO authorizes it in writing. The Contractor is on notice and cautioned that any repairs accomplished without the KO's authorization is done at the Contractor's risk and expense and may not be reimbursed by the US Government. The following factors shall be used to evaluate offers: Specifications on the SOW. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. (End of provision) 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS--COMMERCIAL ITEMS (JAN 2011) An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov.
