Description
The James E. VanZandt VA Medical Center has a potential requirement to furnish all labor, equipment, materials, supervision, transportation, and facilities necessary for pick-up, delivery, set-up and adjustment, instruction on use of equipment, cleaning, repair and storage of beneficiary Durable Medical Equipment (DME) for the beneficiaries of the James E VanZandt VA Medical Center, 2907 Pleasant Valley Blvd, Altoona, PA 16602 (JEVZ VAMC). The Medical Center requires the services of a contractor who is able to deliver and perform the following: 1.Non-tilt over the Bed table, Manual 15" wide by 30" long. 2.Ross Patrol feeding pump with 30 each of 1000ml bags and pump sets 3.IV pole on rolling base 4.Trapeze mount to hospital bed frame with grab bar 5.Hourly labor charge for home medical equipment 6.Base Rate/Round Trip 7.Service calls for emergency Repairs 8.Delivery/Setup or Pickup of JEVZ VAMC owned Telehealth equipment The Contractor is required to follow and comply with accreditations that meet or exceed standards set by Joint Commission. Interested contractors who may be able to perform the services described above are requested to submit a Capability Statement to the Contracting Officer. Capability Statements should include the following information: Contractor's Business Size (Include DUNS number) Contractor's Socio-Economic Category Ability to provide services in the following areas: Bedford, Blair, Cambria, Cameron, Centre, Clearfield, Clinton, Elk, Huntingdon, Indiana, Jefferson, Juniata, Mifflin, and Somerset.
