Description
RFQ CNSHQ10T0005 PRICING SHEET This is a combined synopsis/solicitation for a Firm-Fixed Price (FFP) Commercial Service prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a written solicitation will not be issued. The solicitation number is CNSHQ11T0005. This is a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-47. The North American Industry Classification System (NAICS) is 541380 (Testing Laboratories), Small Business size standard is $12.0 Million Dollars. This acquisition is 100% Small Business Set-Aside and subject to the Service Contract Act. Simplified Acquisition Procedures will be followed. The Corporation for National and Community Service Contracting Office, 1201 New York Ave, NW., Washington DC 20525 is soliciting for Medical Laboratory Testing and Analysis Services in accordance to the Work Statement herein. All firms or individuals responding must have a commercial and government entity code (CAGE Code), must be registered with the Central Contract Registration (CCR) www.bpn.gov.ccr, and the Online Representations and Certifications Application (ORCA) orca.bpn.gov PERIOD OF PERFORMANCE: The period of performance shall be for a base year with three option years if exercised by the Government. (Base - Effective date of award 12 months) SECTION C - DESCRIPTION OF WORK C.1 Introduction/Background The AmeriCorps National Civilian Community Corps (NCCC) is a program of the Corporation for the National and Community Service, a federal government agency. Our mission is to promote civic pride and responsibility to get things done through Corps members work on projects as community service. AmeriCorps NCCC requires an intensive, 10-month commitment. Members serve in teams of eight to twelve and are assigned to projects throughout the region served by their campus. They are trained in CPR, first aid, public safety, and other skills before beginning their first service project. C.2 Scope of Work Approximately Volunteers/Corps Members participate in each Region program with the understanding that drug use is not permitted and that they are required to submit for random drug screenings during their 10 month stay. Thus, a vendor is required to conduct the aforementioned drug screenings for Corps Members monthly. In addition, if probable cause exists during a particular month, the vendor will be requested to conduct additional drug screens on Corps Members, as deemed necessary. Testing services will be located at the following five campuses: Sacramento, CA Denver, CO Perry Point, MD Vicksburg, MS Vinton, IA C.3 The Contractor shall: •(1) Perform a standard 10 panel urine drug screening on Corps Members on site and off site for the following drugs: THC (cannabinoids) Amphetamines Cocaine Opiates Phencyclidine Barbiturates Benzodiazepines Propoxypene Methadone Methaqualone Each month 10-12 members will be tested; in addition to any unforeseen members if necessary. •(2) Conduct drug screens in laboratories located within the United States •(3) Allow for walk-in testing that do not require scheduling appointments. Testing is required during normal business hours (8 am - 5:30 pm) •(4) Follow procedures for handling specimens in accordance with guidelines of the Urine Specimen Collection Handbook for Federal Workplace Drug Testing Programs. •(5) Forward the results in writing or by email to the Corps Counselor at NCCC Atlantic Region within 48 hours, Laura Tuck (410) 642-2411. •(6) Provide access to monthly and intermittent "probable cause" testing •(7) All billable items associated with the individuals drug screen outside of the contract laboratories shall be provided within the bill. C.3.1 Laboratory Requirements •(1) The laboratories must be certified by the National Institute of Drug abuse (NIDA). •(2) The laboratories must be licensed to perform screening according to established procedures. •(3) Medical Review Officer (MRO) services must be performed by a licensed physician •(4) Confirmation testing on all positive specimens in the initial screening process will be conducted by a second method, a no additional cost. •(5) Written confirmation results will be forward to the selected Corps member manager. •(6) The laboratories need to provide proof of insurance "PRICE SHEET PRICE ATTACHED" Offerors are requested to submit a quotation to provide all of the services listed CONTENTS AND FORMAT OF SUBMISSION: Quotation submittal and all other required documents must be submitted in accordance with the solicitation. Failure to submit the aforementioned information correctly shall make your offer non-responsive and will not receive consideration for award. A quotation submitted in response to this solicitation must be submitted electronically and be comprised of the following sections: Section 1: Corporate Overview Company's Professional History in relation to Statement of Work (Company must provide at least years of work history) Company's Facilities and Staff in relation to the Statement of Work Section 2: Company's Technical Qualifications (all below items must be in relation to the Statement of Work) Offerors shall demonstrate the capability to meet all of the following criteria. Offeror shall provide the proposed laboratories where services will be provided. Provide account number, name, telephone number, address of company, and contact person Offeror shall provide evidence of licensing and certifications in accordance with the NIDA Offeror shall state if the proposed facility is currently available to provide services required. Provide 3 names of companies utilizing the facility for same or similar services. Provide name and phone number of contact person. Offeror shall provide name, locations, and contact person of off-site laboratories. Offeror shall identify the types of drugs the laboratories currently test. The offeror shall submit a copy of the organizations most recent independent financial audit or annual report. Section 3: Past Performance •a. Offeror shall submit three (3) references that can attest to the offeror's ability to perform work that is comparable to the work contained herein. Reference information shall include; name of the organization, contact person, current telephone number, address, project number, and if available e-mail address Section 4: Pricing Proposal ELECTRONIC SUBMISSIONS OFFERS MUST BE SUBMITTED ELECTRONICALLY; U.S. Postal Service, Over Night Carrier or Fax submissions shall not be an acceptable form of submittal. All sections shall be sent via one email. Place the RFQ number within the subject line of your submittal. Giles, Contract Specialist CC: and to ljdawson@cns.gov It is the contractor's/offerors responsibility to ensure that electronically submitted proposals are receive by the Government by the due date February 7, 2011 at 3:00 pm EST. Offerors that submit quotes without the quotation cover page will be deemed non-responsive and will not receive consideration for award. •(1) Clarity and completeness of the offer are of the utmost importance. The offer must be written in a practical, clear, and concise manner. It must use quantitative terms whenever possible and must avoid qualitative adjectives to the maximum extent possible. •(2) Offers must be legible, double spaced, typewritten, in a type size not smaller than 10 point pitch with a one inch margin on all sides, on paper not larger than eight and eight and a half by eleven inches and not exceeding the page limits of sixty (60) pages. Pages in excess of the individual limitations shall not be read, and the offer shall be evaluated as if the excess pages did not exist. EVALUATION OF QUOTES The Government will award a Firm Fixed Purchase Order to the offeror submitting the lowest priced technically acceptable (LPTA) offer. Technical proposal will be evaluated on a pass/fail basis for the factors listed below meeting the following requirements. Experience on similar contracts- The Offeror must demonstrate that it has experience with clients in the laboratory testing and analysis services field Technical requirements- The offeror must demonstrate the ability to: •· Offeror shall provide the proposed laboratories where services will be provided. Provide account number, name, telephone number, address of company, and contact person •· Offeror shall provide evidence of licensing and certifications in accordance with the NIDA •· Offeror shall state if the proposed facility is currently available to provide services required. FAR CLAUSES APPLICABLE: FAR 52.212-1 Instructions to Offerors-Commercial Items applies to this acquisition. FAR 52.212-3 Offeror Representations and Certifications - Commerical Items. Offeror must submit a completed copy of FAR 52.212-3 Offeror Representations and Certifications-Commercial Items with their proposal or indicate that the registration has been completed at the ORCA website http://www.orca.bpn.gov. Failure to include this information with the price proposal and past performance may render an offeror ineligible for award.
