Description
THIS OFFICE IS CONDUCTING A ONE TIME BUY FOR THE BELOW DESCRIBED ITEM. THE ORDER IS FOR 300 BOXES OF THREE PART PAPER CONTINUOUS FEED W/CABON LEAVES INSERT. 14-7/8" X 8-1/2" W/ PERPFORATED SIDES ; 1/2" WHITE AND GREEN LINES. ARO FOR DELIVERY IS 10 DAYS OR LESS. PROVIDE SORUCE OF SUPPLY ALONG WITH PROPOSED MATERIAL PART NUMBER FOR CONFORMITY TO THE SPEC'S. THE AWARD WILL GO TO THE LOW BID AS A RESULT F THIS RFQ. ITEM PURCHASE DESCRIPTION General Services Administration Printed on: 1/19/2011, 3:41:15 PM 7530-01-029-1157 UI: BX PAPER, TABULATING MACHINE, CONTINUOUS FLATFOLD: DESCRIPTION PAPER, TABULATING MACHINE, CONTINUOUS FLATFOLD: (3-PART): Shall be in accordance with this description. TYPE - Multi-copy STYLE - Carbon interleaved DIMENSIONS - 14 7/8 inches wide x 8 1/2 inches long per fold. VERTICAL TEARLINE PERFORATIONS - 1/2 +/- 1/16 inch from left edge NUMBER OF PARTS - Three (3) BASIS WEIGHT - 15 pounds, (17 x 22 - 500), +/- 5% tolerance METHOD OF JOINING - Crimping CARBON PAPER - Black; 3/8 inch short on right and left (14 1/8 inches total width) FILE PUNCHING - None FORMS CONTINUITY - Not more than one break per container, splices not permitted. PRINTING - 2 Bars per inch with alternating white and green bars, beginning with a white bar. The width of the bars shall be 12.700 mm (1/2"). COLOR - White SALIENT CHARACTERISTICS Materials: The paper shall be at least 95 percent bleached chemical pulp, with not more than 5 percent unbleached and/or groundwood pulp. The paper shall be tested in accordance with TAPPI T401 for fiber composition. No polychlorinated biphenyols (PCB'S) shall be added or used during the manufacture of the paper or in any coating operation of the base paper. As evidence of compliance, the contractor shall submit a certificate stating that no polychlorinated biphenyls have been added or used during the manufacturing or converting operations. Basis weight: The basis weight of the paper shall be expressed as g/m2 (lbs, 17x22-500). The basis weight tolerance shall be +/- 5%. Basis weight shall be tested in accordance with TAPPI T410. Sizing: The paper shall be suitably sized for writing on with ink without spreading or feathering. Conformance shall be determined by testing. Color: the color of the paper shall be white. Brightness: The brightness shall be 75% minimum carbon interleaved forms. Testing for brightness shall be in accordance with TAPPI T452. Dimensions: The width shall be the overall form width in inches including the marginal aligning strips. The length shall be the distance in inches between the horizontal perforated tear lines. Form tolerances: A tolerance of 1.588 mm (+/- 1/16 inch) shall be allowed on the width of the forms. The length shall not vary by more than 0.794 mm ( +/- 1/32 inch) from the specified length. The length of a strip of forms whose total length is nearest to 304.801 cm (120 inches) shall not vary by more than 2.381 mm (+ /- 3/32 inch) from the specified length of a single form times the number of forms in the strip. ITEM PURCHASE DESCRIPTION General Services Administration Printed on: 1/19/2011, 3:41:15 PM Perforations: All perforations shall be clean cut and sufficiently deep to permit easy separation but do not tear in ordinary handling or in feeding through the machine. Vertical tearling perforations: The location of the vertical tearlines and whether parts and carbons or parts only are to be perforated shall be 1/2 +/- 1/16 inch from left edge, the cut and uncut portions shall ensure straight and proper tears. Crimping: Crimping is a method of joining in which the forms are die embossed or cut in one or both marginal aligning strips thereby interlocking the parts and carbons. Carbon interleaves shall be fastened by at least one prong of a four prong crimp in one margin and by at least one prong of the crimp in the opposite margin. Crimps shall be spaced at not more than 76.200 mm (3 inch) intervals and shall fall not closer than 9.601 mm (3/8 inch) to the horizontal tearlines. Printing: Printing consist of alternate shaded areas as follows: Green Bar Format: Forms shall be printed with alternate tinted light green and white bars as specified, there shall be 2 alternating white and green bars per inch, beginning with a white bar. The width of the bars shall be 12.700mm (1/2"), applicable. The color of the tinted portion shall be such to permit printed numerals or characters to be readily distinguishable on all parts of a form. Carbon Interleave Sets: Multi-part sets shall consist of interleaved with a continuous strips of carbon paper. The number of parts comprising a multi-part set shall be specified. The sets shall be flat folded at the horizontal tearline to produce the number of multi-part sets. Forms continuity: Each strip shall have no more than one break per shipping container with not less than 100 continuous forms in any broken strip. Not more than 10 percent of the shipping containers shall contain broken strips. No splices shall be permitted. Workmanship: The forms shall be clean and free from dirt, foreign matter, tears, wrinkles, creases, folds (other than at the horizontal tearline), paper dust, lint, or grit or other defects which adversely affect appearance or serviceability. Labeling: The recycled content shall be marked on the product or unit pack. PACKING: 950 sets shall be packed in a close-fitting fiberboard box. The pack shall be compliance with the Uniform Freight Classification and the National Motor Freight Classification and shall not be more than 51 pounds in weight. MARKING For civilian destinations - In accordance with FED-STD-123H, dated August 8, 2007. For military destinations - In accordance with MIL-STD-129P, dated December 15, 2002, and Change 4, dated September 19, 2007. Bar code marking shall be included for both civilian and military destinations. PALLETIZATION IN ACCORDANCE WITH BELOW PALLETIZATION REQUIREMENT, DATED 8/15/2004, UNLESS OTHERWISE INDICATED IN THE CONTRACT OR PURCHASE ORDER. PALLETIZATION REQUIREMENT 8/15/2004 ITEM PURCHASE DESCRIPTION General Services Administration Printed on: 1/19/2011, 3:41:15 PM Palletization: The shipping (transport) containers shall be palletized. The pallets shall be 48-inch length x 40-inch width, general purpose, four-way entry, flush stringer, and double-face nonreversible pallets. The palletized load shall not exceed 2500 pounds in weight for shipments to the Eastern Distribution Center (Burlington). The palletized load shall not exceed 2000 pounds in weight for shipments to the Western Distribution Center (French Camp and Stockton Locations). The palletized load shall not overhang the pallet deck board edge by more than 1 inch and shall not exceed 53 inches in overall height (including pallet). Less than half pallet loads or loads shipped by small package carrier in acceptable transport packing need not be palletized. When less than full but more than a half pallet is used, the palletized load shall utilize proper bracing and/or reinforcement to ensure that the load can withstand two additional loads placed on it. Stacking: A load-stacking test shall be performed once, at the beginning of the contract, for each different unitized or palletized load configuration. A fully palletized load shall be tested utilizing a total stack of three palletized loads, with all the proper bracing and/or reinforcement necessary to perform the test. There shall be no evidence of damage to the pallets, shipping containers or products when stack is tested on the rigid surface for a minimum of 24 hours, in compliance with all the National and Local Safety Regulations. The pre-tested palletized load configuration shall be duplicated using the same packaging materials, pattern, number of shipping (transport) containers, and procedures consistent throughout the duration of the contract. Palletized loads shall be stable and safe when handled with mechanical equipment by qualified personnel. Materials: All lumber used inside or outside a shipment shall be bark free. No used wood pallets with repaired components or stringers will be acceptable. All wood pallet stringers shall be manufactured from hardwoods. Pallets manufactured of materials other than wood, shall conform to all the requirements specified herein. Unitization: For the purpose of this requirement, the assembled group of containers or items in a single load that can be handled as a unit throughout the distribution system. Unitization encompasses, but is not limited to, consolidation in a container, placement on a pallet or load base having forklift capability, or securely binding together. Shipments should be considered for unitization where appropriate or specified. Unitization of pallet load: The palletized load shall be unitized and secured to the pallet to insure that the load arrives intact without product damage. The containers or items shall be placed in a suitable pattern to form a stable and balanced load. Vertical, horizontal or filler reinforcement shall be added as necessary to meet the stacking strength requirements. A palletized load shall be secured to the pallet by shrink or stretch wrapping, and/or, by steel or nonmetallic strapping. When steel or nonmetallic strapping is used, edge protectors shall be provided to prevent damage to the cartons when tensioning the strapping. As a minimum, two straps shall run lengthwise, two straps shall run widthwise, and two straps shall run around the girth of the palletized load. A palletized load shall be capable of protecting the products against damage in a multiple handling, transportation, and storage environment having distribution warehouses with rack systems that support the load/s overhead of warehouse personnel. Notice of special requirements for shipment to all countries that have endorsed the IPPC Guidelines for treatment of non-manufactured wood packaging: The International Plant Protection Convention (IPPC) has approved and published on March 15, 2002, Guidelines for Regulating Wood Packaging Material in International Trade. Countries endorsing the IPPC Guidelines are found at http://www.aphis.usda.gov/ppq/swp/. Clause D-FSS-468 requires nonmanufactured wood pallets and other non-manufactured wood packaging material used in shipments destined to IPPC compliant countries require the appropriate wood treatment. PCM REGULATORY REQUIREMENTS: The contractor shall certify that the product offered shall contain a minimum of 30% recovered fiber, including 30% postconsumer fiber in accordance with the EPA Comprehensive Procurement Guidelines, which may be found at: www.epa.gov/epawaste/conserve/tools/cpg/index.htm The Government reserves the right to require proof of such conformance prior to first delivery and thereafter as may be otherwise provided for under the provisions of the contract.
