Solicitation Number: VA-241-11-RQ-0234 Notice Type: Combined Synopsis/Solicitation Synopsis: This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. All offerors MUST be registered on www.ccr.gov, MUST provide a Valid DUNS Number and TAX ID Number, along with a detailed pricing and delivery quote. If any of the items are not sent along with the quote, the response may be considered non-responsive. (ii) The solicitation number is VA-241-11-RQ-0234 and is issued as a Request For Quotation (RFQ) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-35, July 14, 2009. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 339112, and the size standard is 100 employees. (v) This requirement consists of the following items: All surfaces that the blood flow comes in contact with have to be thrombo resistant via heparin coatings. There are two manufacturing companies that make these coatings: Medtronic and Maquet. Medtronic coating brands are Trillum and Carmeda. Maquet coating is Bioline. There are different components in the perfusion circuit. 1.Cardiac Perfusion Pack to include the following items: a. Oxygenator b. Arterial filter c. Venous reservoir (bag-closed system) d. Cardioplegia set e. Cardiotomy for valves f. Centrifugal blood pump g. Custom tubing pack that connects all of the components 2. The following items do not have to be heparin coated: a. In-line blood gas sensor and venous saturation/hematocrit b. Cerebral oximetry c. Blood cell salvaging and processing device (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: The Department of Veterans Affairs, West Roxbury Medical Center has an immediate need for a supplier of Cardiac Perfusion Packs. Orders will be called in as needed and need to be shipped the same day. All offerors must state their capability to have these items in stock at all times. Offerors need to specify the days/hours orders are accepted and specify how orders are delivered (Fed-EX ground, AIR, etc.). Offeror MUST specify shipping costs, if no shipping costs are listed in the response, all quotes will be considered FOB DESTINATION. Offeror must specify the minimum/maximum amount allowed per one shipment. This request may be set aside for Small Business, Veteran Owned Small Business, or Service Disabled Veteran Owned Small Business. The contracting officer reserves the right to award without discussion, based on the evaluation criteria below. Lowest vendor technically acceptable (vii) Delivery FOB Destination to VA Medical Center, 1400 VFW Parkway, MA 02132-4927. More information will be provided upon contract award. (viii) 52.212-1 Instructions to Offerors - commercial Items, apply to this solicitation with the following addenda, (ix) 52.212-2, Evaluation - Commercial Items: FAR provision 52.212-2 applies to this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Lowest vendor technically acceptable. All evaluation factors other than cost or price, when combined, are approximately equal to price. (x) 52.212-3 Offeror Representations and Certifications - Commercial Items- Offeror's are to include a COMPLETED COPY OF PROVISION 52.212-3 WITH THE SUBMITTED QUOTE, unless offeror has a current ORCA certification. A copy of the provision may be attained from http://www.arnet.gov/far; (xi) 52.212-4 Contract Terms and Conditions - Commercial Items: apply to this solicitation with the following addenda; VAAR 852.203-70 Commercial Advertising, VAAR 852.246-70 Guarantee, VAAR 852.273-76 Electronic Invoice Submission, and VAAR 852.246-71 Inspection.
