- Register of Wage Determinations Under the Service Contract Act 05-2103 SF-1449 Statement of Work 1. Purpose The purpose of this Statement of Work (SOW), is to provide the Federal Bureau of Prisons with forms automation support for policy-related documents and other documents mandated by the Director and/or by court action. 2. Point of Contact As identified by announcement. 3. Contract Type and Schedule Contractors will be used on an hourly-basis rate, not to exceed a total of 1980 hours per year. The term of this contract is a base year plus four option periods. Base Period: Date of Award through September 30. 2011 Option I: October 1, 2011 through September 30, 2012 Option II: October 1, 2012 through September 30, 2013 Option III: October 1, 2013 through September 30, 2014 Option IV: October 1, 2014 through September 30, 2015 4. Place of Performance 400 First Street NW, Washington, DC 20534 and in the Mid-Atlantic Regional Office located at 302 Sentinnel Drive, Suite 200, Annapolis Junction, MD 20701. Contractor may also visit NARA locations in Washington, DC and College Park, Maryland. Short-term travel assignment within the continental U.S. may be required. 5. Task Description I. Specialized graphic design and electronic drawing tailored to the creation of legal instruments (forms), including the programming of logic, math calculation, and input configuration/validation rules to be applied to form or to each form update action. Tasks also include: • Creation and maintenance of electronic forms including scanning, field population, artwork designs, field operations automation, validation and other programming controls, among other functions. • Converting for files to Adobe's.PDF format and add fields and logic to these.PDF files. • Post files on Internet/Intranet server as needed • Participate in meetings and design sessions with other departments to review forms conversion and workflow technical details, as available. • Design and create electronic form files in WordPerfect and Adobe.PDF formats. • Use HTML and JavaScript code to provide programmatic logic to files and to post the files for agency-wide access on the Intranet and Internet servers. • Support MS Access databases by creating queries, reports, and other support files. Contractor will help maintain these files in the branch's servers. • Perform related forms and document conversion/automation tasks as assigned. • Assist with the management of policy-related paper and electronic documents vesting forms, including records management supporting tasks as assigned. 6. Program Management The contractor shall provide a Point of Contact (POC) who shall be responsible for the delivery of forms automation under this contract. The POC shall have the full authority to act on behalf of the contractor in all matters relating to the daily operation of the contracted language translation support services. The contractor shall designate this individual in writing to the Contracting Officer prior to the start-date of the contract. An alternate POC may be designated; however, the contractor must identify those times when the alternate shall be the primary POC. The contractor will provide a monthly billing statement to the Contracting Officer that accurately reflects the number of hours worked each day and the total dollar amount due for that month's services. Payment will then be made monthly upon submissions of payment vouchers by the contract monitor or their designee. 7. Minimum Requirements/Qualifications/Skills I. Office Automation Software Skill Set a. The contractor must have at least three (3) years work experience with electronic forms design. This experience must include: i. Microsoft XP ii. Adobe Life Cycle Designer ii. Adobe Acrobat Professional 7 or higher iv. JavaScript v. MS Access vi. HTML publishing tools such as Team Site and/or MS FrontPage II. A four-year degree in computer graphics design or a closely related field is required. 8. Validation of Credentials and Technical Qualifications Prior to award of this contract, the contractor must provide resumes of the proposed personnel offered in support of this project. The resumes will be used to evaluate the contractor's qualifications for the job. The agency will have the right to interview and test the skills of the prospective candidate to evaluate the experience and knowledge in areas specified in this SOW. The agency shall have the right to review and qualify these resume credentials. The agency will qualify the contractor based on the review of the contractor's candidates, resume, and/or oral interview. Technical qualifications will be based on the evaluation process described herein. In the event that an contractor employee is replaced, or otherwise leaves during the contact period, replacement personnel will be evaluated using the above specified criteria. The Bureau of Prisons reserves the right to reject replacement personnel deemed to be under qualified. 9. Method of Evaluation The Federal Bureau of Prisons anticipates making a single award contract based on a technically acceptable/lowest price source selection. This will be an "all or none evaluation." Quotes will be required to meet all criteria specified in the attached statement of work in order to be considered for award. The evaluation factors and significant sub-factors that establish the requirements of acceptability are set forth herein. Award shall be made on the basis of the lowest evaluated price of responses meeting or exceeding the acceptability standards for non-cost factors. Tradeoffs are not permitted. Offers are evaluated for acceptability but not ranked using non-price related factors. The Federal Bureau of Prisons may make award without discussion, however reserves the right to open discussions if deemed necessary. Quotes will be evaluated in the following areas: 1. Technical Acceptability 2. Price 3. Past Performance Past Performance: The contractor shall include in their quote response, a minimum of three (3)past performance references. Each reference shall include a valid point of contact and a telephone number, fax number, and email address for that individual. Responses that do not include this information will not be rated on Past Performance 10. Faith-Based and Community-Based Organizations Faith-Based and Community-based organizations can sumit offers/bids/quotations equally with other organizations for contracts for which they are eligible.
