- Background The Richard L. Roudebush VA Medical Center, Indianapolis, IN requires certification, decontamination, inspection; filter changing, maintenance, trouble shooting, repairs and parts for approximately 37 bio hazard hoods, 41 fume hoods, and 9 clean bench machines. In addition there is an annual requirement to do performance testing and certification of our Surgical Suite and testing. This is a set aside contract for small business and shall be a base year to begin January 1, 2011 with four 1 year options to follow. NAICS code is 541990. Offerors are responsible for obtaining a copy of all documents (solicitations and amendments) via the website: http://www.fbo.gov without further notice from the VA and for monitoring the website for up-date information. Offeror will submit their proposal along with past performance, references, certifications, training, and copies of sample test and certification reports. Scope Contractor will furnish necessary labor, materials, supplies, and equipment for maintenance inspections and certifications, (Decontamination, and changing of filters, when necessary) of biological/biohazard cabinets, fume hoods and clean bench/laminar flow stations as identified in the attached equipment listing identifying fume, bio and clean bench numbers and location. Objectives To provide services that will guarantee a safe working environment, uncompromised medical and research formulation and processes. Maintenance, testing and minor repair to equipment so as to provide as much uninterrupted or delayed use of equipment by healthcare professionals, research and laboratory technicians. Also provide various reports, certifications, and testing results to document integrity, performance and industry standards that are required by owner/user, and various oversight and regulatory agencies. Tasks ANNUAL AND SEMIANNUAL PERFORMANCE TESTING OF BIOLOGICAL SAFETY CABINETS, LAMINAR FLOW WORK STATIONS AND CHEMICAL FUME HOODS. THE PURPOSE OF THE WORK IS TO INSURE THE CORRECT OPERATION OF THE EQUIPMENT WITH REGARD TO PERSONNEL, PRODUCT (RESEARCH) AND ENVIRONMENTAL PROTECTION. PARAMETERS TESTED INCLUDE, BUT ARE NOT LIMITED TO AIR FLOW BLANCE AND ADJUSTEMENT, HEPA FILTER INTEGRITY TEST TO 99.99% FILTRATION PERFORMANCE, AIRFLOW SMOKE PATTERNS BY USING PROPER TESTING EQUIPMENT, ELECTRICAL TESTING, LIGHT AND SOUND LEVELS. DOCUMENTATIONS OF ALL DATA AND ACCEPTANCE CRITERIA IS PROVIDED WITH REPORT AND PERFORMANCE CERTIFICATION. ALL BIO-CABINETS I, II, III THAT HAS HEPA FILTERING EQUIPMENT MUST BE TESTED ACCORDING TO MANUFACTURE SPEC. TO INCLUDE THE STATEMENT OF WORK. VENDOR SHALL NOT USE STEAM TO DECONTAM BIO-HOOD OR CABINETS. DECONTAM SHALL BE DONE BY USE OF THE FORMALDEHYDE UNLESS LOCATION FORCE THE OPERATION TO AN EVENING OR WEEKEND. Medical Center mission of patient care and availability will on occasion require performance of testing and or services to be performed off hours, weekends, etc. and emergency response with 24 hours. Also, scheduling of work will have to be made because patient care. This will not incur undo cost to the government. Delivery 1. In case of emergency an on-site response time of less than 24 hours is required. 2. Certifications/reports: Tests of Biological Safety Cabinets will consist of, at a minimum, required physical test as outlined in annex F, (National Sanitation Foundation) NSF/ANSI 49 -2009 edition. Tests of Laminar Flow Clean Benches will (at a minimum) conform to IES RP-CC-002, parts 4, 5, and 6. 3. All test equipment must be calibrated within the last 12 months to an NBS traceable standard. Copies of Calibration certification must be available for review by VA as requested. 4. Technician (s) performing the work must be qualified by education, training, and experience. They must have had the appropriate training for certification of biological safety cabinets, be an NSF accredited certifier. (Current copy must be on file with VAMC). 5. All certifications and reports must be delivered within two weeks upon completion testing and contain relevant information regarding, equipment type, class, location, testing parameters, space configurations, velocity profiles, air flow calculations, filter leak test, decontamination, culturable bacteria test results, and all other pertinent information specific to industry acceptable practices for each devise and or area tested. Reports to be delivered to the Contracting Officer Technical Representative (COTR).
