Description
Statement of Work, Pricing Chart and Instructions to Offerors This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; no written solicitation will be issued. Solicitation No. DTRT57-11-Q-80017 is issued as a Request for Quote (RFQ). This RFQ is being conducted under Simplified Acquisition Procedures (SAP), FAR Part 13 and FAR Part 12. The incorporated document and provisions are those in effect through Federal Acquisition Circular 2005-46 dated October 29, 2010. The NAICS Code is 562112, Hazardous Waste Collection, and the Small Business size standard is $12.5 million. This RFQ is being Set-Aside for small business. The U.S. Department of Transportation, Research and Innovative Technology Administration, John A. Volpe National Transportation Systems Center (Volpe Center), Cambridge, MA, is seeking to purchase commercially available on-call response, containment, identification, removal, disposal, and clean-up of hazardous materials (HAZMAT) from the Volpe Center's buildings and grounds. HAZMAT may include but is not limited to: packaged chemicals (commercial products and lab chemicals), wet cell batteries, paints, coatings, adhesives, solvents, and lubricants, etc. Provide suitable containers for routine collection of HAZMAT, remove and replace as needed. During the period of performance the vendor shall provide trained personnel, all equipment, supplies, and facilities necessary to respond to, contain, identify, remove, dispose of, and clean up hazardous materials from the Volpe Center's buildings and grounds. The vendor shall provide suitable containers for the routine collection of hazardous materials and remove and replace them as needed. FAR Provision 52.212-1, Instructions to Offerors -Commercial Items (JUN 2008) is hereby incorporated by reference. Under FAR 52.212-2 Evaluation-Commercial Items, award will be made to an offeror that provides a quote that meets the Governments requirements and has the lowest price. NOTE: All contractors must be registered in the Central Contractor Registration (CCR) in order to receive an award from the Government. CCR registration information may be found at www.ccr.gov. The Offeror's signed offer shall be prepared as either a PDF or Microsoft Word file document and submitted electronically via email to Donald.MacGee@dot.gov to arrive no later than 2:00 P.M., Eastern Standard Time, on December 10, 2010. The quote package must be marked RFQ: DTRT57-11-Q-80017. Quotes submitted via facsimile or U.S. Mail are not authorized and late responses will not be considered. The Government will not pay for any information received. Solicitation Provisions: All Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (OCT 2010) must be submitted with the offer, an offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete paragraphs (c) through (m) of this provision. FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items (JUN 2010), is hereby incorporated by reference. FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (OCT 2010), is hereby incorporated by reference, additional clauses cited in 52.212-5 that apply to this acquisition are: 52.219-6, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.222-50, 52.223-18, and 52.232-33. This RFQ also incorporates by reference FAR 52.216-22, Indefinite Quantity, 52.217-5, Evaluation of Options, 52.217-8, Option to Extend Services, 52.217-9, and, Option to Extend the Term of the Contract. This RFQ also incorporate as follows TAR 1252.223-70 (DEC 1997) Removal or disposal of Hazardous Substances-applicable Licenses and Permits. The Contractor has does_____ does not have ______ all licenses and permits required by Federal, state, and local laws to perform hazardous substance(s) removal or disposal services. If the Contractor does not currently possess these documents, it must obtain all requisite licenses and permits within 5 days after date of award. The Contractor shall provide evidence of said documents to the Contracting Officer or designated Government representative prior to commencement of work under the contract. Attention: The following notice is provided for informational purposes for Minority, Women-Owned and Disadvantaged Business Enterprises. The Department of Transportation (DOT), Short-Term Lending Program (STLP) offers working capital financing in the form of lines of credit to finance accounts receivable for transportation related contracts.
